Some of these glitches remind me of this one incident I had on the first GBA Castlevania. There were some red crates that needed a special item to allow you to push them, and for some reason, I was trying to push one without the item. While I was pushing, I jumped, and I think the game detected me as sandwiched between the ceiling and the crate. The game then threw my character fully across the screen, BROKE a torch as I passed over it (this only can occur from attacks, not general movement), and threw me half way across the screen in the next room. I didn't die, or even take damage, but it was amazing to see on a 2D game. At least finding map holes on a 3D game is a little more explainable. I never was able to reproduce the incident.
One EPIC death I had was while playing Driver. A cop sandwiched me between a wall at full speed, and sent me flying up 200+ feet into the air. It was high enough that the draw distance in the game stopped showing the ground. Finally, after reaching the height of my accent, I came back down, almost right where I had been when I crashed. I managed to land on the hood of another car that was just minding its own business as it drove around.
