I'd go with Resident Evil 5, when i put down my controller to watch a cinematic causing me to miss a quick time event and then watched Albert Wesker drive his hand through my chest(near the end in the airplane).

Most of the deaths on Dead Space 1 & 2, the regenerator comes to mind mostly.

Well I've only played the first Dead Space, but yeah I've got to second this. That game doesn't just let you die, it really rubs your face in it by killing you in the most prolonged and graphic way imaginable. Esp. that fucking hive mind. It's like, 'Ok, I get it. You've eaten me, I'm dead. Can we skip to the reloading of the last checkpoint please?'

Far Cry 2. I was getting really pissed off by being inst-killed by cars around the time. A car was speeding towards me, and I killed the driver and the car stopped in front of me. I got really happy, then I got run over by some other guy who just drove up behind me.
