For me it's Dimitri in Grand Theft Auto 4. It's just soo well done, in the short time you get to know him I really did feel I had formed a friendship (as much as you can in a game that is) and there was trust so when he double crosses you it actually felt like a kick to the private parts, and it's that, that fueled me to complete the game and exact my revenge. Though the game kinda slumped in the middle due to poor missions and a mehish storyline about diamonds when all I cared about was killing a certain Russian but this is turning into a review so i'll shut it.

I'm going to have go with a literary example here. If you ever want the socks scared off of you, go pick up a copy of Karin Lowachee's Warchild. The Big Bad, Falcone, is easily the only fictional character to actually make physically nauseous with how disgustingly evil he is. And every single time the main character encounters him, Falcone's in complete control. Very scary, very disturbing.

Evil Otto from Berzerk, sends shivers down my spine.

He is the only video game villain to actually kill the players in real life. He can also be drawn in MS Paint.

Evil Otto first appeared in the 80s in the game Berzerk, the plot to this game was rather impresive, featuring epic battles of a Green man (The Player) shooting Yellow robots (The bad guys). If the Robots shot you, you die, if you touch the robots, you die, if you touch the Cyanide laced walls, you die. It really was quite fun.

It was Otto's job to fly through the evil death walls and destroy the player when the timer ran low. At first, Otto was just a pain, untill he actually killed to players in Real Life! Between 1981 and 1982, two teenage Berzerk players died of heart attacks shortly after posting high scores at video arcades. Evil Otto watched them die... with a smile on his face.
