Actually, how about a list?

1) Civilization 2: easily over 100 times
2) Age of Empires 2 (singleplayer): over 70 random map games finished
3) Delta Force 2 (singleplayer): about 63 times (one mission per time)
4) Civilization 4 (singleplayer): 46 times
5) Call of Duty: Modern Warfare (campaign): 31 times played through
6) 1701 A.D. (or Anno 1701)(singleplayer): 23 times
7) Cossacks: Back to War: 21 times
8) Halo: Combat Evolved (singleplayer): 5 times
9) Fallout 3: 3 times
10) Assassin's Creed: 3 times

Ocarina of Time: I used to speedrun it, so I've beaten it over 200-250 times. I used to beat the game every day for an entire summer, at approximately 5:15/playthrough. Sometimes more, sometimes less. Personal record was 4:58, but TSA had the record of 4:57, so I couldn't edge him out.

Majora's Mask: About 30-40 play throughs.

Diablo 2: At least 30 times (beating Baal on Hell), but that's online, which makes it much less of an achievement.

Pokémon games: About 15 times, but with countless hours logged.

Not sure which one I've sunk the most time into, either Pokémon, WoW, or Ocarina of Time. I think OoT is first place, then WoW, then Pokémon, but I could be wrong. I doubt I am though.

halo CE and 2 for sure are the ones i've replayed most, i know the cut scenes word for word and just about everything inbetween. then rainbow 6 vegas 2 for sure then probably black, i've spent days of my life on that game.

Ocarina of Time is probably my most replayed game. I make a point of replaying it once every couple months because it is that good.

I always seem to go back to a Silent Hill game, any of them, except for the very first though.. sadly, because it is quite outdated today.

That, and I'm not above going back to EverQuest just to reminisce on a wonderful gaming past. My friends and I played that game quite a bit back in the day, and it was the first online 3D world. Continents, lore/history, big dragons, it was quite amazing. Of course, that whole model has been pounded into the dirt by now, but it's still nice to revisit the game from time to time, or even EverQuest 2 as it shares the same lore.

However, if it's a game that I go back to the most, it's the whole of the Silent Hill series. I wouldn't really count a MMORPG as a contender in this, because it's just something you log-in to from time to time.

Probably the Saints Row games. I'm not sure if I prefer one over the other, but I love the campaigns of both. I can't stop creating a whole new character and playing it differently than the last time.

Have also played all the Zelda games multiple times

CS 1.6 has been my universal for ever.
For ever...
Since before I had it I logged hours upon hours of game time at internet cafes in Europe...
I've logged more hours on three seperate different counters than I have on any other combined games.
Plus I'm awesome at it, which gives it bonus points.
Er, in second place would be GTA San Andreas, simply because I redownload it each month after looking at the different mods that people make for it.
I've had so many different variations of that game it's not even funny.

Severance: Blade of Darkness. I've played through that game so many times, although I didn't complete it that many times when I was younger I still kept coming back to it.

Actually, ironically enough, I have a hard time picking a game to just "go play again" when I'm bored. There are so many that I love to play over and over.
