I'd wager Devil May Cry 3: Special Edition still holds that title with me. 80+ hours with Vergil alone.

Notable competitors would be:

Pokémon as a series,
Assassin's Creed as a series,
and GTA IV.

Can't get enough of those games.

I think Terraria. 207 hours so far. Then again, I beat Saints Row 2 about 5 times, and 100% it twice, so maybe that game.

I think it's Guild Wars 2 now, 1043 hours, probably about tied with Final Fantasy XII, Rome Total War and Medieval II Total War. Age of Mythology and GTA Vice City could be close contenders as well.

Top spot goes to The Legend of Zelda: Ocarina of Time, easily. I beat it close to thirty times when the N64 was in its prime, and I still occasionally go back to it. I've got to have clocked close to 1,000 hours there. Don't regret any of it: no other game can elicit the feelings that OoT does. Best thing ever.

Second place would probably be to Final Fantasy X. If I had to guess, I would say that I put near 500 hours into it. All of my playthroughs clocked in at around 90 hours, and I beat it about five times, so you do the math. If I still had a PS2 (or my PS3 had backwards compatibility...), it would be more. If the HD remake ever sees the light of day, playtime there will definitely skyrocket.

Rounding out the top three would probably have to be World of Warcraft. I haven't played in months, but I'm pretty sure I would have clocked in around 200 hours total, on all characters. I didn't play it much, but when I did play it, I played it for long stretches of time. I'm actually kind of glad I got out, because I do regret that time spent a little bit. And the money...

Honourable mentions other than that would have to go to The Elder Scrolls IV: Oblivion, Super Mario World, Dragon Age: Origins, and Demon's Souls. So many fond memories.

Well... I've completed Kingdom Hearts II over twenty times, and at least seven or eight of those have been playthroughs where I try to get 100% so they'll be 50-60 hours per playthrough... So probably that.
That said, though, the first one was longer thinking about it. I'm sure I have playthroughs of 100+ hours on that one, but not as many.

Over the years, Baldur's Gate 2. I played it ALL YEAR on the year it released. I've played it for a few months of each year since. I'm willing to hazard saying that I've beaten the series as several variations of every class.

But DOTA2 will overtake BG in a relatively short span of time. I've sunk over a thousand hours into DOTA2 just since August last year, and I'm still honeymooning. It's the best game I've ever played, in all regards save narrative.

Either Halo 3 or Borderlands 2.

For a while there, I was the number 1 player of Borderlands 2 on Raptr. I can't be sure if it's still like that, as I've gone to play my 9 other yet-to-be-started games for a bit, but yeah. It was a lot.

Halo 3, I have no clue, as I wasn't on Raptr back then, but it was very consistent every day for a good a year or so.

Not sure, but I'm guessing Dark Souls. At least a good 150-200 hours on the console version, add another 30 to that for the PC+PtDE content.

Either:

Pokemon Diamond (around 300 hours last I checked the file)
WoW (Raptr says some absurd number that I don't believe, but it's still probably near a few weeks worth of hours by now)
Original Harvest Moon (used to play the hell out of this game when I was a kid)

My two most played games?

Pokemon soulsilver-spent a very long time grinding my pokemon to fight the end boss X-X but it was so, so worth it! I even have a feraligatr in black/white 2!

Dawn of war: Dark crusade- Love 40k, and Dawn of war helped me get even more of it!

Steam clock alone says 1831 hours for Counter-Strike: Source. I played it before the counter, and now I've clocked 250 hours on Global Offensive. No idea how much time spent with 1.3 and 1.5 back in the day.

Mine is probably Wolfenstein 3D, but I have no way to tell exacly how much time I may have spent with that.
The game I have the most tracked hours, though, is TF2, with about 650h.

Honorable mentions would be:

Pince of persia (the original) ? certainly a few hundred failed playthroughs and about 50 successful ones
Tony Hawk pro skater 2 ? playing with friends every other day for about 2 years
Age of empires 2 ? Playing it consistently ever since the expansion came out
