Family inheritance... a very nerdy family inheritance. WAAAAAY back when I played Everquest my dad's character was Aarthek, and he eventually convinced me to put down Quake and start playing a character. Instead of having me make a randomized name or some mindless non-sense, he gave my character the name Zarthek, it's been with me since.

I used to be a moderator at the Axalon, a UK-based Transformers fansite (don't bother looking, it doesn't exist anymore); they had a really cool rpg going there and I named my character, which I modeled after Soundwave, Sonicron (as in sonic waves).

I was playing TF2 with a group of my friends and I had a bad internet connection and my friends kept complaining about my lag so out of rage I changed my name to Captain Lag because I usually get sarcastic when I'm mad. And I thought it was awesome.
