First let me exclude certain parameters so this doesn't become an "I hate x game" thread, this is a moment when you were playing and disaster strikes in the form of "Holy god what the fuck happened to me?"
Or it could be the obligatory Dark Souls/Demon's Souls accidental dodge off a cliff...
My example:
Playing through a map on Battlefield 1942 (one of the Pacific theater maps) I was all gung ho over a hill to take a base, seconds from a cap and I get used as a landing strip by a damn kamikaze. This after surviving multiple rounds of enemy fire and shelling from various tanks.
Turns out it wasn't the enemy, it was one of my teammates who jumped out of the plane by accident and it just slammed straight into my body.
Fun times.... but that was definitely one of the worst moments of gaming.
EDIT: I did see a guy later that game get lucky and escaped an exploding tank to walk right into a shell from the enemy ships... good times.

Mariokart 8. I've won the last few races in first place in the 150cc Thunder Cup. Going smoothly. Last corner.

Get hit by Blue shell.
Then red shell.
Then slip in mud.

I actually remember waaaaaaaaaaaay back when I was still younger playin Diddy Kong racing for N64.
That last level where you have to beat that pig..boss..thing. I played it for hours on end and for the life of me could not beat him.
I was in first..Ill say for 90% of the bloody race and saw the finish line infront of me. Then that..pig passed me. That was probs the first rage quit I can remember. I just stared at the screen for about 2 min and then turned it off and went outside to play haha.

On the Black Ops II zombie map tranzit, my friends and I spent around 2 and a half hours building the jet gun, which turned out to be pretty useless, and I died with it about 5 minutes after we built it. We were very upset about that.

In Halo 3 on the map Valhalla I was using the spartan laser to preemptively destroy the banshee that had spawned in the other team's base. I was on the hill in the middle of the map, aiming the laser through the middle of their base right into their banshee's spawn. Just as I fired, a teammate got into their banshee, our banshee did a flyby on their banshee in an attempt to blow it up, and our team's warthog went off a ramp as it was trying to escape from the enemy base. The stars aligned and my spartan laser managed to hit ALL OF THEM. 4 betrayals in an instant. Thankfully I was playing in a full party of friends so they thought it was funny, and 6 years later they still haven't let me live it down.

It's a shiv up, but for me. I'm going with my NES playthrough of Metal gear the original (not MSX unfortuno'), i must've been, say, seven? Possibly six. -evsies, i had played the game three or four times before, and even traded it away in furious anger (the sort only a child knows at feeling like they're not beeing told something), i had finally made some headway ,through sheer stuborness and rigorous trial and error i had made it all the way to the second part of the complex, and then i was called away for something vital, i 'magine it was either Darkwing duck or Centurions.'member those?),no?) so i jotted down the password, paused, had my brother AND my mother write it down. And shut it off.

Came back an hour and a half later.

You know the twist, the 'words were dead, a hidden apocrypha on parched pavement for all the use it was.

And it was on that day that i knew insanity and cruelty, and welcomed R'hlyehs inevitable awakening.

I had some very poor games in Age of Empires 2 and Age of Mythology. Being swamped by enemy cavalry before I'd even begun to orientate myself, that kind of thing. I was never a fast RTS player. I much preferred to build a secure little town before I thought about striking out.

I also have a moment from Guild Wars 2 SPvP in my head. Usually I kick ass on the Skyhammer map: You can step on this fragile glass to weaken it, and a moment later it breaks; the best way is to step on, step off, and then use a pulling skill (like the engineer's Magnet or the thief's Scorpion Wire) to pull an enemy onto it just as it shatters, and they'll fall to their death. On this occasion I rounded a corner, stepped onto the fragile glass, and was immediately hit with an Immobilise. So, all I could do was stand stock still and wait for the glass to break, and career down to my rocky demise.

(On a more fun occasion, myself and an opponent were both desperately trying to push eachother off a floating platform. My opponent managed to push me off, but in the split second before their skill hit, I activated my Magnet. I was pushed off by his skill, and he was pulled after me, and we both died. It was magnificent).

So far my experience with Dark Souls is "go through starter dungeon, beat boss, step into real world, get gang-banged by a bunch of angry skeletons, repeat, repeat again, repeat a third time, repeat a few more times on top of that, ragequit, plan to come back to it after beating the rest of my gaming backlog"

Also, getting GTAV on my 360 right about the time the motor on the disk drive started to go. It was frustrating having to spend half an hour hoping the drive would get up to speed when I started playing, then praying it didn't konk out during gameplay.

There was a game...called Seventh Saga ...I think it was for the SNES system (one of those). This was along time ago...and for whatever reason I had been playing it all weekend long (not sure if I rented it back then, if I had just gotten it and fell in love etc). I never saved it...because I never stopped playing it. Anyway, after playing non-stop for 20+ hours...getting near the end of the game....someone (Don't even remember who), blew out the power with a vacuum cleaner.

There where no auto-saves in the game....and so all my progress was gone. I was crushed. I did replay it all over again, and a few times more after that as other characters...but at the time I was very upset.

I've had lots of bad experiences....other times when the power went poof and I lost progress....games not working past a certain point (bad cd etc)....but I remember that time the most..even though it's been like 20+ years since it happened.

It did teach me to save occasionally when playing games at least:)

You're going the wrong way. Stay out of the graveyard, you don't have the weapons or the experience necessary to survive it. You should be going the opposite direction, into the aqueduct (it's on your right when you get to firelink).
