[HEADING=1]It was a sunny, crappy day, in San Andreas...[/HEADING]
And i was chasing a portly fellow with a dildo, down the highway.

Long story short: i got hit by a semi.

a funny fail was when me and my buddy were playing halo 3 co-op campaign, and we were trying to sneak around these enemies (for some reason we were whispering, thinking that would make us sneakier)

to sneak around them we had to go over a barrier, we were crouch walking and I turned to my friend in the game and said "this is what we are going to do" as I said this he was slowly sliding down the side of a cliff, that he couldn't walk back up , i gently whispered "K see ya" and he fell of the cliff to his death

we both ended up in fits of laughter, it was kind of a 'you had to be there moment' but i tried explaing as best i could

I believe my FAIL was in...

CAll OF DUTY: Black Ops/ playing Team deathmatch on the Grid map
I was walking through the map looking for a kill to boost my killstreak for the last 3 minutes of the game. Then finally i found a guy, after he shot a bullet through my teammate skull, on the floor fighting for his life when he had a "last stand". So i walked towards the guy in a slow motion,(with music playing in background: Jaw's theme music ) taunting the guy that im about to kill him, when i saw there was only, about20 seconds in the game, i rushed over him only that the had just thrown a knife in the air and hit a metal beam, and came rushing down on top of my head just in time for a final kill!

Surely this can't be all the fails we've got, come on Escapists, let's hear 'em!

Unknown Source: Ooh, that's really, really unlucky, but I'd say that more of a fail on his part for just throwing it randomly. :)

Aside from getting lost all the damn time in Fallout games, or most open world games. It would have to be missing obvious things when moving from point A to B or when solving RPG puzzles.
