I think I have the problem sorted out now. Antialiasing seems to have been the culprit. My fps went from 5 to 60 with occasional lag spikes because GTA 4 is poorly optimized.

Ok, so I messed around with the Nvidia control panel and turning the settings down helps a lot. I still get small lag spikes but it seems to be working fine now.

Yes, but even when i turn the settings way down, it doesn't make a significant difference. My PC is super high end and should run it just fine.
