The irony that this thread is right above 'would you torture someone' does not escape me.
Anyway, GTA San Andreas, my friend built up a barricade in a train tunnel. He got out and waited. I said "I hope a train doesn't come." The train came immediately afterwards-from the side my friend was on. He was pinned to the cars until the train derailed. He actually didn't die until everything went boom, courtesy of some oil barrels. We laughed our asses off.

Battlefield games have given me many good deaths, generally along the lines of, low on life, hobble to a safe place, plan my course of action while i realise I am in the middle of shit, then proceed to get out of said shit and be hit by some bastard I didn't see (this happened alot in battlefield 1942, sometimes after a long hair rasing crawl through hell).
Another more recent one would be in the STALKER series:
A. 'Look a shiny artifact', runs to get it ZAP! unseen lightning anomally, dead cause they do that.
B. Low on health, Huffing and puffing sound of the scariest bastard I have yet to meet (creature that goes invisable), where is it? where is it? right in front of me, bam, it attacks and blinks out of sight again. Few shots fired before the second fatal attack. That thing scared me shitless every time until I had played the game through 4 times.
Another more recent one would be in the STALKER series:

I've died many ways in video games. Perhaps the most frustrating to date would have to have been in World of Warcraft: Death by Felreaver in Outlands. The things are gigantic metal monsters and yet somehow manage to sneak up on you. By the time the ground starts shaking, it's already behind you and pretty much just walked right over you when you were at level for Hellfire Peninsula.
Death by Canon in Serious Sam. The original, mind you, where you simply turned into big, shapeless chunks of meat and blood smeared over 40 feet of grass in a nice skidmarky fashion.
Urgh, don't remind me, it took me SO long to figure out you needed to use that pendant >.< As for in-game death, how about death by that 1st Goomba in Sumer Mario Bros? I think we've all been killed by it at least once.Midnight Crossroads said:

A few seconds of falling later, I hit a tree branch and somehow went shooting almost vertically down along a road.
A few more seconds of bouncing on the road, some people and a wall or two, I actually manage to survive by some miracle and get back up.
Then I get run over by an ambulance and die...
I can only tolerate so much irony without throwing a fit of laughter.
