I have the same problem. I played through the whole game with the same 2 companions without switching once and spend 50+ hours doing almost every single side quest and I still didn't get any on that list
Then my girlfriend-at-the-time played through it and got one of them by simply rushing through the main story under 20 hours. Insane and ridiculous
I had the EXACT SAME PROBLEM. People keep telling me I have to do all the side missions and I FUCKING DID. Maybe not on EVERY playthrough I've done, but on the first 2 at least. Then my girlfriend played it twice and got the achievements on both of her playthroughs, and she doesn't even give a shit about gamerscore. I'm considering scrapping all of my characters and starting from scratch again.
Maybe it needs a woman's touch
...definitely the most retarded thing I've ever said, but what else could it be?! This can't be sheer coincidence!

I managed to get every single flag in Assassins Creed 1, pretty proud of myself.. but Oh wait.. it decided to glitch out the templars so I have one missing. 970/1000 >:C

Oh, and I recall trying to get the "Guardin' gnome" (I'm pretty sure that's the right name) in Left 4 dead 2 with a friend of mine, went through most of the campaign with that damned gnome then as the helicopter in the finale landed the horde overran us and we couldn't be assed to try that part again.
Thank you. I was looking through this thread trying to think at what point it was when I decided the urgent, irrational need to get achievements ruins games. Now I remember.

I have the achievement. Several hundred times over. That's just the thing. I can't think how many times i've escorted that accursed gnome and his eyes-of-a-serial-killer stare through that campaign. The first time I did it, one other idiot decided it was a perfect time to go for the confederacy of crunches achievement for going through a campaign only using melee weapons. Very helpful. Now, every time someone tries to free that little cretin, I wait for the doors to his wisely locked cell to open, then chuck him over the nearest fence.

Eugh, I hate achievements and what they're doing to gamers. If the achievement is for something that I was already trying to do, then hooray for my e-peen, so basically I only have 100% completion on games that are actually fun enough that it's worth doing (which aren't many).
