1. Level design not making sense
I blow a hole in a wall via some means, and behind it is a storeroom. With no doors, windows, or vents. What did they do? Build the room, stock it, then brick it over? How much extra effort would have it taken to add an in-operable door? Or a goddamn room shoved off some precarious catwalk, which can only be reached via jumping from another catwalk, are workers expected to do this daily?
2. Shitty Elevators
Because I just love being stuck in a unimaginative whirring metal coffin for a minute. Elevators are actually a fond love of mine in games, I take needless rides on them just because, especially if they have the cheesy music to go with it. But they need windows, or at least grating to look out of at the skyline/shaft/whatever. Bonus points for complex shafts. Box elevators that probably don't even actually move just tarnish this love.
3. Bees!
OH GOD THERE STINGING MY EYES! Basically, swarm type undamage-able enemies. Acceptable when they're only released by your own damn stupid fault. (SS2 Coop, bastards kept setting off the swarm pods, then we all screamed beeeeesss! and ran around like idiots till the things died of fear or something.)

5. No difficulty adjust/Can't change difficulty mid-game/unlock-able difficulty
Hate when I can't change the difficulty, I like to play on easy first, just to enjoy the game at first, then switch to the hardest later on. Hate when easy turns out to easy, and I have to restart to change it. Also, why do I have to play the game so many times just to play on the hardest setting, unless it's a setting called RADISH ADVENTURE, in which case it's a good thing :shudder:
Absolutly loved the difficulty settings in System Shock, could set it so the whole game was on a time-limit, enemies repopulated like they were spliced with rabbits, took a shitload of shots to kill, and puzzles were practically impossible; or so that all doors start unlocked and no story elements are present, everything dies in a single hit and is non-hostile, puzzles don't exist, and things never re-spawn. That level of customization made up for being unable to change it.
