Kane And Lynch, personally, wins in terms of "didn't live happily ever after" just because you get two endings and either way it's a bad ending.

Either you leave Lynch to burn and your daughter hates you, or Lynch gets shot and your daughter possibly killed
Even if it got horrible scores, I love that game, the whole dark tone of it is one of the reasons why I loved it.
Same, and I can't help but think the multi-player held absolute tons of potential.

Fallout 3 Vanilla.
The "good" heroic ending would be the one in which YOU die.

Most of humanity, along with a huge part of the universe has been exploded/disappeared, the UMN(think super bad ass version of the internet), which is absolutely crucial to life has been destroyed preventing FTL travel, the hero/love interest blew itself up and is floating in space, the protagonists brother is just flat out dead, the protagonist has to travel to Lost Jerusalem despite the loss of FTL travel and not having much of an idea where in space it actually is, and as the final punch to the player's stomach the rest of the universe has an extremely good chance of kicking the bucket as well in a few thousand years(you killed the person who was trying to stop that from happening).

Nobody has mentioned FF: Crisis Core? That game had a sad ending, that you KNEW was coming, but I'll be damned if I didn't spend 40 minutes at the end trying to fight fate lol. As the final battle went on, my heart kept slowly falling as I realized I couldn't cheat the game and get an uncanonical ending. Regardless of seeing it coming, by giving you pseudo-control of the final moments I believe the ending was masterfully executed. Nothing gives you a connection to something like that like putting the futility of the situation into your own hands. Especially since by then you are virtually a planet wrecking god if you did all the extras.

And just a side note... Zack is by far a better character than Cloud. :p

Knights of the Old Republic 2: The Sith Lords, has a very similar ending to Planescape: Torment. Mostly because they were written by the same guy.

Neither of the endings in KOTOR 2 are very happy. Despite all your efforts, all you have managed to do is prepare the galaxy for the next Sith onslaught. And judging by the events in the upcoming Old Republic MMO, it looks like even that failed.

In Kreia's own words. "Perhaps you were expecting some surprise, some information that eluded you, that would make things clear, define your purpose. No, there is only you."

It has such deep meaning, but at the same time it undermines all of your "accomplishments", by instilling a sense of futility. All your actions in an attempt to save the galaxy were only tools in Kreia's plan to show you who you really were.

What about a bad good ending?

I.E., saying your character committed suicide by a bomb he/she strapped onto his/her body, then ran up to the villain, killed him/herself and the villain?

You never played Persona 3: FES did you?
You save the world from the god of death, but all your party lose their memories of the entire battle. 1 month later Aigis the robot girl, the only member of the party who remembers, comes to school to keep the promise you all made to meet up after graduation and your party suddenly remembers what they went through together and run to the roof. Aigis is up there cradling the main character's head on her lap while she tells him how, now the battle is over, she will live to protect him. And... then he dies just as his friends all arrive, having expended all his life to save the world a month earlier.
The expansion story "The Answer" softens the blow slightly, but only after twisting the knife.
Dammit I was going to say this.

Other than that P4 ends a on a bit of a sad note with the MC leaving after saving everyone's bacon and the song that plays there Never More just adds to the effect.

Probably said by now, but Half Life 2 Episode 2.
Halo 3 and Halo Wars both had a "Victory with a price" theme going on
Fallout 3 without Broken Steel;
die
Those are a few

Not really a video game, but there will be only 3 possible(cannonical) ending for the WH40k unniverse:
-Chaos completely takes over, and all hell breaks loose(literally)
-The Necrons seperate the materium from the immaterium, litterally destroying every soul in the galaxy. And then they harvest everyone to feed their Gods.
-The Tyranids completely wipe everything out, turning the Milky Way into a lifeless void of barren rocks.

No matter what happens, the "good guys" (and I use the term very loosely in 40k) will be dead.

I admit, I bit my lip at the end of FFX.

GTA4 is pretty rough no matter which ending you get. Either way, Nikko gets hosed.

The Darkness ending seemed pretty sad to me.

Final Fantasy Tactics made it feel like everything I did was for naught.
Aw man, The Darkness . . . I completely forgot about that one, talk about sad . . . that shit sucked hard, but he still managed to do what he set out to, so he still conquered the situation at hand, even though he has a "TRAGEDY TO OCCUR IN 10 . . ." hanging over his head.

Really there is ONE exception, there was a happy ending I found truly sad.

Anyone here ever played Klonoa? either version, Wii or PS1 . . . seriously though. 25+ hours into that game, hit the end with every possible collectible . . . FINALLY kill the last boss, the ending rolls, he saved the world, congratulations, and leaves off for his house.

Sun rises . . . he wakes up with the ring next to his bed (the ring he uses throughout the game) and the spirit inside badmouths him for sleeping so late and he says (at least in the PS1 version) "oh wow . . . it was all a dream?" I seriously fell to the ground.

Damn you!

...I was going to mention Crisis Core...

That scene at the end was the ultimate form of controllable helplessness in my eyes, I mean, you've slain the most powerful summons known to man in one-on-one combat, captured said summons to do your bidding, and on top of that, slain a facking God, and regardless, Zack is still utterly defenseless against those three Shinra troops that come out from behind those rocks...

Damn you SquareEnix for making such an unbelievably awesome character! Damn you to hell! *insert Manly Man Tears of Manliness*

That's not sad, that's just lazy! I absolutelly despise the "it was all a dream!" crap. Heavy Rain did it too, and it doesn't work any better.

Any of the Final Fantasys. How are the heroes supposed to re-adjust to normal life after what they've been through? Everything would seem so pointless and mundane. I'd be willing to bet that Zell'd hang himself.
That was kind of the premise for FFX-2. In fact, I think some of the early dialogue had Yuna commenting on how boring the Eternal Calm was. Same with FFVII: Advent Children.

Not necessarily to endorse FFX-2 or anything, I haven't played that game, but I think the fact that they were able to make a whole game out of that premise shows that, happily-ever-after or not, people are always going to have problems.

I prefer a sort of sad/happy ending. As in a ending that is happy, with sad elements mixed in. Anyone who's finished Lost Odyssey knows what I'm talking about. Or Kingdom Hearts (1st one) for that matter...
