I know most people here don't like FF7 but for me the death of a character that surprised me the most was Aerith. No way in hell did I think until then that it was possible for a controlable character to die!

Nicole Horne from Max Payne.
First her helicopter can crushed by an antenna, then the chopper falls 100 stories from a skyscraper taking the landing pad with it only to be crushed by the landing pad and then set on fire.

Ming the Merciless' death in Flash Gordon was awesome.
He gets impaled on the front of a rocket ship as it crashes through the wall of his palace.

Derek Reese. After everything our main cast has been through, he gets popped in the chest without even firing back. Yes, the Derek in the future is still alive, but it's sad to know this is how he meets his end. From the same show, Charlie. God this show loves killing off its characters.
Ned Stark. Come on, this one was a shocker. After reading so much light fantasy, I didn't realize what a total subversion Martin's books were until Stark was executed. I kept thinking, "But he's the main character!" There are no main characters. There are just people, and any one of them can expire at any moment.
For me probably Louis from Resi 4, its so funny what the tenticle looks like that kills Louis. heres a link for those who haven't seen it
