Ever played Deus Ex lately? It's combat system was not anywhere near flawless too.

Vampire the Masquerade: Bloodlines. It's awesome and everything a computer RPG should be.

Runner up: Deus ex, Baldurs gate 2

While not exactly Flawless (there's a serious imbalance in the augs) I think it's better than any other FPSRPG system I've played. It handles the conflict between player skill and character skill well, if you are untrained with a gun you won't be effective with it but you can still use it. It encourages a more tactical approach than other similar games.

Say for example you have a shotgun and are untrained with it, you'll die in a straigt up fire fight but if you ambush an enemy at point blank they die, contrast with say Fallout 3 where skill determines damage instead of acuracy and you can shoot a guy point blank in the head with a shotgun and they'll be almost unhurt. This mean if you're using a weapon you're uskilled with in Deus Ex you have to use it more creatively whereas in Fallout 3 you simply have to use it more.

The Witcher. This is mostly because it diverted quite a bit in tone from my previous RPG favourites(Neverwinter Nights, Morrowind and to a lessr degree Bloodlines).
Dragon Age and New Vegas are also up there, but there's a certain lack of...spark for those two.

KoToR. It won me over. After being hesitant to play it due to all the hype I finally sat down with it...for hours and hours. It was fantastic, shame the sequel had so many flaws.

Also I'm a JRPG fan so I like my battles turnbased but KotoR was enough of a middle ground for me to enjoy it, so yeah definitely my favourite western RPG of the last 10 years.
