Best: Psycho Mantis and Gray Fox from MGS1.
Worst: Any of the Final Fantasy children from either Kingdom Hearts game, specifically Wakka from KH1. I'm not a fan of small children to begin with, but his voice was just...ugh. Hated it. Was glad when the typhoon tore up the island.

Space Rangers 2 had atrocious English dialog. I don't think some of the actors spoke English at all- they were just reading the lines and hoping for the best.

Gears of War voices drove me insane. The dialogue was just awful and having to listen to that macho marine/solider talk made me want to mute the game. I mean does ever game like this have to have the babysitter, the guy whom always tells the others to knock it off and get in line, the dumb black guy who carries a big gun and talks $hit at 30 words per minute, the wisecracker who usually makes the stupid jokes and either has red hair or a thick accent and/ or the rookie that they can make fun of or a butch woman that they can crack sex jokes towards.

Dude - it's a completely macho game where you chainsaw aliens and take lots of steroids. Of COURSE it's going to have dialogue like that. But anyway, you're describing dialogue rather than acting, and you see that the voice actors have gone for powerful characterisations, surely suggesting that their voices at least fit.

Best/Worst award goes to Ron Perlman

Best (or at least pretty good) - Fallout Intro "War never changes..."

Worst - Conan. Ronny, could you sound like, I don't know, a BARBARIAN!

Heavenly Sword and Metal Gear Solid have some of the best voice acting I've ever heard.

Patrick Stewart from Oblivion was phenomenal, but everything else in that game fell short.

Mass Effect and the first KOTOR had brilliant voice acting.

EDIT: Oh yeah, and how could I not forget the aweful voice acting in The Longest Journey. Too much swearing, patronising characters like the cleaner outside the cinema, the flying guy with no legs. It doesn't help that the dialog was droning and longwinded also.

As for best, just about anything Tim Schafer is involved with.

The worst would be the Necron Lord in Dawn of War: Soulstorm. All he does is say one syllable words and than gasps for air.

I actually don't mind MGS' voice acting, it's actually pretty good(story, though. oh dear)

Good:
Starcraft(especially zeratul and tassadar), all of the characters have awesome Voice Acting
Morrowind's characters have pretty good voice acting too and all somehow seem unique
Half life

honorable mention:
Okami's gibberish actually fits the game well
Disgaea's was terrible but the characters and humour(for me) transcends that

Bad:
too many for me to mention

Best Hands Down: Armin Shimerman, who played Andrew Ryan in Bioshock

Worst in my opinion: Whoever played the voices of all the Elves in TES 4 Oblivion.

For me, the best video game voice over would have to be Joe Kucan from Command & Conquer. He's the bad guy, but he does such a good job of making you think otherwise, especially since he puts so much emotion into it.

My worst? Hmmm... The little squeakers that you hear over Xbox Live and any other online network.

Maybe it was just the load time on the audio files, maybe it's the actors themselves, non the less, the voices on that game REALLY pissed me off.
