Joining a MW2 Free-for all about ten minuets into the twenty minuet match, and getting second place, dueling with the guy who was point leader when I joined.
The game winning kill was him missing me and hitting some poor sod sneaking up behind me.
Had I hit HIM, I would have one the game.

When I beat the Doom Dragon in Golden Sun: The Lost Age. I know I stand virtually alone on this one, but the Doom Dragon was a lot harder than Dullahan
When I beat Luigi's Mansion my 2nd time. I found king Boo IMPOSSIBLE when I was a kid. Now I whipped without even taking a hit of damage

A blitzkrieg battle in MAG.
Me and my squad were on valor and attacking SVER. We were one of the APC squadas and we got through the burnoff, cooling and captured and held the objectives(for awhile) in less that 10 minutes. We steamrolled them destroying AA and bunkers and mortars and Motor pools. IT WAS EPIC!!

I've had a few, but one of the ones that has always stuck with me was on Pokemon Blue (back in the old days)
I'd made my way to the elite four, I knew my pokemon weren't strong enough to win, but I decided to try it out, knowing that if I failed I at least had experience points for my pokemon so they would be more likely to make it.
So I made it all the way up to land and thought "Holy shit, I'm actually doing this!" When I won I was rather in shock, it was truly "OMG I WON!"

Bioshock 2, Online in deathmatch, i had no health and was getting my ass kicked by a big daddy who was tieing with me for 1st there was ten seconds left of the match i went to pick up a rock with telekinesis and by chance picked up a grenade shot at me from a turret which came out of nowhere and killed the big daddy and won the match!!
While I have had a few, for me the most memorable was while playing Breath of Fire 2, where I defeated the final two bosses with the shit characters.
Maybe not the most difficult thing, but it was certainly satisfying.

Condemned had a pretty difficult final boss that I remember feeling particularly satisfied after beating. Same goes for Dead Rising.
Getting Tidus's weapon after doing those infuriating races is definitely up there. My swearing was heard throughout the house every time I came a second or two too slow.(Dents in the wall as well..)Gauntes said:
Best accomplishment I can remember is after my guild in WoW nearly wiped during phase three of Onyxia(after she was remade for level 80s)I was the only healer remaining alive with our Pally tank. We were about to give up when one of my guildmates jokingly said that we could two man it with an OP Pally tank and an OP Druid healer. We thought this was pretty funny and decided to give it a go and somehow it seemed to work.
Solo healing Ony for 28 minutes(Thank god for no enrage, also for Innervate, Lay on Hands, and Brezzing)is definitely one of the most frightening things I have done in my life so far.(Everytime she did her knockback my heart skipped a beat) It got easier when I was able to brez one of our other druid healers and he brezed a dps, but it was a definite accomplishment. Sadly, I didn't even win anything when the loot came up for rolls.
I'd have to say that my biggest shock victory was in Baldur's Gate 2, fighting the dragon that's just before the ending (the one you HAVE to kill in order to get a quest item). In that game, the dragons were an all-or-nothing proposition, and the only really solid strategy I'd found for defeating them was to use Feeblemind and hope they failed their save.
Well, it didn't work, and it was mauling my party while I desperately tried to come up with plan B, and then, boom, the dragon suddenly vanished. Apparently one of my mages Disintegrated it. While I wasn't looking.

The biggest recently for me was beating the madden moment in madden '10 where you have to do the game winning drive with the lions. Actually didn't convert to 2 points for the win but on the following kick off I forced a fumble and the scooped and scored.
