My biggest "OMG I WON" is completing all 50 states in the original Rampage on the NES on a single playthrough, followed closely by the biggest "slap to the face" by the ending cutscene.

It's not a glitch. There's a 2-up just before a drop off in one of the factory levels right after the start of the level. Grab that, continue.
