The "Kessler Reveal" from Infamous 1. It's like Sucker Punch was trying to make fun of Infinite Crisis, but it still hit me hard.

Yeah and depending on how you play it he may or may not come back. It's pretty brave.

In Soul Nomad and the World Eaters, the revelation of where the souls of the World Eaters came from, the main character's past (it's an especially big deal if one picked the female version of 'Revya') and what the main character really is are all high voltage shocks, more so than the merely surprising true identity of the current god of death, Gig or finding out who Endorph was(which likely only has significance to anyone who played Phantom Brave).

The biggest twist is that one of the alternate endings sets up the main story itself.

WYK from BioShock or Kessler's finale from inFamous (the story had been so basic up 'til then that it left me feeling lost).
But the one that left me the most flabbergasted was Tales of Symphonia's triple-plot-twist of Remiel, Kratos, and Yggy. By the third fight I was practicaly hysterical.

For me it had to be the proper ending of Nier, though i know most of you probably didn't play or tolerate the game long enough to get the final ending, But any game that makes you delete all of your save files and watch as your inventory is destroyed deserves a special mention.

It blew my mind watching 40 hours of my life get deleted.

The twist near the end of Amnesia: The dark descent, where you find out why your character erased his memory. Perhaps not so much a twist, but it was definitely an eye-opener.

Phoenix Wright: Justice for All's last case has a pretty surprising twist to it. Heck, pretty much every 'last case' of all the Ace Attorney games have some big surprises to them. Of special note I think (going "sequel number of game"-"case number") 1-4, 1-5, 2-4, 3-5, and AAI-5.

When I first played it, KotOR definitely threw me for a loop.
It took me a while to wrap my head around it.

System Shock 2's plot revelation was the biggest at the time (1999).
Bioshock rehashed most of the plot lines.

I know it's been said ~ 500 times but KOTOR. No game has shocked me as much before or since. It blew me away, and the way they went back and pointed out all the little hints that some of the characters were dropping... Completely blew my mind. (Jade Empire would have been great, but TV Tropes ruined it for me... stupid hi-lighting things indiscriminately)

I second Mass Effect 1's revelation of Sovereign... I'll also pull out two of them from ME2

1- When you find out the collectors are actually that extinct species from ME1
2- When we finally find out what the collectors were doing with all those humans
(Instead of keeping them for food, or prisoners)
