Biggest "whoops" moment in gaming?

Back when I was playing halo 3...
I was snipeing some guy, and when I finally was going to hit him, my thumb slipped off the damn analog stick and I some how sniped myself.
Confused I looked in theater mode and saw that the bullet bounced off one of the walls...then I just failed.

Saints Row 2: I was in a attack heli with 5 stars, when a black smoking police heli pops up in front (Missle I shot at it before must have not killed it). I could have just fired a couple of bullets out of my mingun. Hell I could have nudged it out of the sky, but I went for overkill. I fired my rocket without it fully logged on. Right round the police heli and on it's return trip it missed by an inch. Leaving me the last thing in it's way. BOOM. Mission FAIl

spyro 3. had won the game and was chasing moneybags for my cash. every time i hit him, half of my precious little gems fall into oblivion.

CS:S. was at a game-night (12 hours of gaming with 25 other people). me and my buddy oscar(think it was him) "ninja plants the bomb in dust2 on A. planting normally then pushing the nearby barrel over it, making it almost invisible. my friend budd sebastian shoots us both, but runs in circles trying to find the bomb. at last he finds it, stands on the barrel and defuses, but it explodes, sending him to the moon. we cracked.

CS:S. some idiot firing at me when i run around the corner to check for enemies. when hes on the way to the CT camping the other way, i shot the CT through his head.

In LOTRO Mines of Moria I was in the waterworks trying to kill a boss I needed but he was well protected and it was hard to get to him without getting a bunch of adds. A champion close by asked if I needed him and said he would help. We fellowshipped and pulled the boss along with about 4 adds. He was dpsing and I was healing. Then my batteries in my mouse went completely dead! The poor guy and I both died and I couldn't even type until I got new batteries installed. I explained what happened and he was gracious enough to try it again after I removed his dread.

Playing split screen multiplayer in James Bond: Nightfire for the Gamecube, also known as the best first person shooter(to me).

On a level with two bases, one some sort of clock tower the other...something else. Bro decides it's fun to play as Odd Job. Uses the sniper to find me on the opposite base, switches to hat, and throws. I, having my own sniper, see this and run back and forth and jumping in order to avoid the heat-seeking-one-hit-kill hat. Eventually, it came close enough that I could see it, and I jumped straight into its path. Unable to change directions while jumping, I died.

On the same level, I hide in a corner while reloading only to find said brother finishing reloading his shotgun right next to me.

My friend and I were playing Halo co-op. We were driving a Warthog at full speed, and we got to a checkpoint only to realize we were driving toward the edge of a cliff. We couldn't turn or get out in time before we went over the edge, and then when we respawned, we were heading at full speed toward the cliff again, unable to get out. We kept dying over and over.
