I found the bridge puzzle in Dragon Age quite frustrating before I put pen to paper and made notes on which combinations did what. Was really easy then.

You must have been playing the PC version. From what I read, the puzzle was dumbed down in the console versions.

Perseus from GoW2 on Titan mode. I kept trying over and over then I fell asleep, woke up the next morning and tried a few more times before finally beating him.

FFX seymour atop mount Gagazet
He's the one that gets 2 attacks in a row, he'll zombie a party member, then full life them for an instant kill. He instantly kills anything you summon, and has a physical attack that hits all party members for lethal damage if they dont have enough HP (or protect). Reflect helps since he's a zombie and his life spells will hurt him when reflected, but he has a mass dispel to get rid of it along with your protect. The zombie status is applied with a physical hit so it cant be reflected which means you have a bunch of zombied party members (cant be healed) getting pummeled with insanely powerful hits and instant killed if either of their protective spells wear off.

All that could be forgiven, it's a nice complex fight after you die a few times and figure out whats going on. The problem is there's a 10 minute unskippable cutscene right before the fight that is pure AGONY to sit through 3-4 times.

there's a really great Lets Play of it done by some goons ***MILD SPOILER ALERT*** if you really care about the plot of FFX and havn't played it yet for some reason http://www.viddler.com/explore/kuvo/videos/52/

I'd also like to nominate Nyx from Persona 3. That guy has like 12 forms and is a fucking cheap-ass! I still haven't beaten the bastard, every time I think to go back and try again my blood boils at the mind numbing frustration of it.
Her Night Queen move is basically the middle finger without Narcissus' flower or other status blocker items.

Speaking of P3,it pisses me off to no end that one bad turn can fuck you up.also the fact that if the MC dies the game is over.

I'm tempted to say Ninja gaiden and leave it at that but for some specifics, any battle where the bow is required,Murai and Ghost Doku for being a cheap ass health stealing mother-fucker.

Flying enemies with projectiles in Star Ocean 4, they basically ripped through my magic users and kept my character and other attacker pinned down while they destroyed our health bars.

EDIT: Also the Meat Circus from Psychonauts between the little Ollie escort and and your dad's obstacle course, I really wanted to kill something before the end of it.

demi-fiend in Digital Devil Saga

you literally have to gimp yourself if you even want to stand a chance. That's not to say i dont like the boss, its a really well-designed boss, just REALLY hard!

The Meat Circus level in Psychonauts was frustrating as hell. It left a permanent scar on an otherwise fantastic game.

The final boss in heavenly sword. The rest of the game was pretty easy, but that mothefuckers wings made it impossible to see what kind of move he was using, so I couldn't block. Maybe it's just my shit tv, but arrgh!!
No, it's not your shit TV. That guy is just a dick, and so is the person who programmed the boss fight. I don't rage-quit often, but that boss did it for me.

Every time I think of annoying puzzles, I keep going back to the symbol puzzles in Assassin's Creed II.
Yeah, the first 8 or so were fine, but after that it got so frustrating at times it made me want to rip my teeth out.

On-topic: Devil May Cry 3's Vergil. You fought him 3 times, and damn was he hard. Especially if starting at normal difficulty without going through easy first.
While I have to agree that Vergil was difficult to beat, especially in the final fucking fight, he never made me rage-quit because he was cool as all hell, which kept the fun alive for me. Seriously, Vergil was one of the best characters in any game ever and fighting him rocked, plain and simple.

Also, thinking of annoying puzzles... there's this one puzzle in God of War 1, towards the end when you're high up in the Cliffs of Madness. The concept was mind-numbingly simple, but it was a timed puzzle that insta-killed you upon failure. You had to drag a box out of an alcove and shove it through a room and across a corner, jump on it and use it to reach a high ledge; However, once the ridiculously short timer ran out, spikes would shoot up from the floor, pulverizing the box and turning you into Kratos Burgers. That puzzle nearly made me break the controller in half.

Gow Spike Puzzle
That one pissed me the hell off too. What infuriated me more then the puzzle its self even was having to watch that one fucking second cutscene showing the door locking. One second doesn't sound like much but after watching it a few dozen times because you keep dieing because kratos doesn't jump high enough or far enough by ONE FUCKING PIXEL and the camera perspective is fucking weird as hell in that room, it's most definitely rage inducing. I GET IT, DOORS LOCK WHEN I PULL LEVER STOP PLAYING THIS
Yeah, the repeatedly playing cutscene was the icing on the sewage-cake. And I agree, while the fixed camera works well most of the time, it's especially annoying in puzzles like this one.

Level 110 Chaos in Dissidia: Final Fantasy. That fucker just pulls these massively damaging combos on you with no indication so you need to have fucking ESP to block them, and his HP attacks are friggin impossible to dodge, even Jecht Block doesn't work on the most annoying one (that one where he hits you into the air with some magic swords and then drives you into the ground with a spear).

I got really fucking frustrated and I actually buggered my PSP, so now I have to send it away to get fixed again.

The final bosses from Kingdom Hearts 2. Bored to to fucking death and the plot still seemed to be unfinished. That was probably because I didn't do all the extras but still. Also the helicopter from the first Syphon Filter is so tedious and repetitive and annoying, and if you screw up one of your mores that you have to repeat over and over then you have to start again, repeating the same moves over and over for another five minutes. Oh and the boss from Duke3D in "The Birth". The Queen I think it's called. Jesus that was hard on any difficulty other than piece of cake.
