The final boss in heavenly sword. The rest of the game was pretty easy, but that mothefuckers wings made it impossible to see what kind of move he was using, so I couldn't block. Maybe it's just my shit tv, but arrgh!!
No, it's not your shit TV. That guy is just a dick, and so is the person who programmed the boss fight. I don't rage-quit often, but that boss did it for me.

Every time I think of annoying puzzles, I keep going back to the symbol puzzles in Assassin's Creed II.
Yeah, the first 8 or so were fine, but after that it got so frustrating at times it made me want to rip my teeth out.

On-topic: Devil May Cry 3's Vergil. You fought him 3 times, and damn was he hard. Especially if starting at normal difficulty without going through easy first.
While I have to agree that Vergil was difficult to beat, especially in the final fucking fight, he never made me rage-quit because he was cool as all hell, which kept the fun alive for me. Seriously, Vergil was one of the best characters in any game ever and fighting him rocked, plain and simple.
Also, thinking of annoying puzzles... there's this one puzzle in God of War 1, towards the end when you're high up in the Cliffs of Madness. The concept was mind-numbingly simple, but it was a timed puzzle that insta-killed you upon failure. You had to drag a box out of an alcove and shove it through a room and across a corner, jump on it and use it to reach a high ledge; However, once the ridiculously short timer ran out, spikes would shoot up from the floor, pulverizing the box and turning you into Kratos Burgers. That puzzle nearly made me break the controller in half.
