For game characters I used to use Azoroth until i found out that that was WoW related, the i started using Azuron or Azmodeon, mostly just with an Az in it. For accounts i always use standokan.

In fantasy type games, I usually go with Azarias (Which was originally Arias) or Everius. I've got a few other variations depending on the character itself but those are the oldest ones I have.
Edit: o_O Standokan and me apparently have similar taste in names.

Not that original, but if it's a character I created, meaning it don't already have a default name, I'll often go with Bob. Because Bobipine would be somewhat redundant. I'm trying to go with other names, but can't think of many more original things. I think I went with Jarate for fun once.

I generally use the names:
Norick, Amide, or Mosie

If I want a female-centric game I usually pick the name Anouk.

Whichi - it was a nickname given to me from a friend way back in grade school, so I thought I'd use it on various game characters.

Masaharu - I hung out with this one Chinese girl (And by hang out, I mean I had to share a table with her in Study Hall) during high school. I was making a Monk for D&D 3.5, and I didn't feel like using the same name, so I asked her "if you had a baby boy, what would you name him?"

Mortran - A friend used that name for any and all dwarven melee characters. I upheld that tradition.

When I feel like dicking around I rely on my friend Richard Johnson if it fits into the name generator. He is my rock band character among other things.
Also, oh god the pun.

and i use names from history during my dnd campaign

Captain Dick (as in Dick Tracy). Does not work very well when there is some kind of profanity filter (I wonder why? :p)

FPS i'm always called Sc1enc3 thanks to an old clan nickname, for RPGs im always called impavide, it's my family motto and it means fearless, thought it was pretty badass

Jarcin. Don't know where it came from, don't know why, but it started with my Forsaken Mage and worked from there.

Always Fred. If it's for something online where Fred will obviously be taken, the name will still have Fred in it.

I don't know why I like the name Fred so much, It's not even my own name.
