No kidding. That and the Ancient Prophecy. Why does there always have to be an Ancient Prophecy? Why can't things just happen because they happen?
Because, otherwise, the Ancient Mystical Cults would be out of work. And it explains why you must go on an epic journey to find the Ancient and Mystical Plot Device! ;)

Short brown hair, and grizzled-ness.
You two must be clever.
What is that supposed to mean?
It means: quoting someone who is funny does not make you funny. Stop doing it. It's annoying.
Urgh be quiet, I was intentionally quoting it as a reference.
Yes. And do you see how eighty other people also did it? We've all seen Yahtzee's videos. We don't need to reminded of what he says in every thread. Stop. Doing. It.

In-game female lead characters confusing 'strength and independence' with 'being a *****'.

Sometimes not even the lead character. Like that CIA (if I remember correctly, anyway. I think she's CIA.) woman you rescue from the school fairly early on in Crysis who replies harshly when Psycho refers to her as 'love'; something anyone with any understanding of colloquial UK English would know basically is used in the same way (in that context) as 'guy' would be. (The female version of 'mate' to remain on colloquial UK English).

it's impossible to have any narrative without a trope anyway, so don't bother trying. [http://tvtropes.org/pmwiki/pmwiki.php/Main/TheTropelessTale]

Exploding barrels being left everywhere, and stupid henchmen standing in big groups around them. You'd think they'd learn.

Well sometimes it is down to developers choosing to focus more on gameplay than the story, stereotypes and cliches are usually included as they are an easier way for someone to identify a character and how they may fit in with the story, without the developer having to write reams of back story for the world the game takes place in. Like orangebandguy says people do like familiarity and patterns.
