Well there's Boyd from Psychonauts. There are other characters in that game that are just as if not more crazy than him, but he and his level, The Milkman Conspiracy particularly stand out to me.
You could count almost every siginificant NPC from the Borderlands series, but one reigns supreme over the others. He only appeared in one location, never did anything, was not siginificant to the plot or anything for that matter, but he was still the most memorable character of Borderlands 2. And the craziest.

Obligatory Joker from Arkham Asylum/City reference. Though I'm not entirely sure he's nuts. He balances the fine line between planning something intense and batshit bonkers.
