The Brood Mothers in Dragon Age were horrifying, not only because they were pretty terrifying enemies to begin with, but hearing how they're created and having that slow build up to that insane monstrosity that was once a woman, it was sufficiently creepy.

Also, there's a lot of insanely creepy monsters and moments in Legend of Zelda: Majora's Mask. I know a lot of people love to talk about how 'dark' Twilight Princess is, but Majora's Mask just had me feeling like I was Alice lost in Wonderland.

One of my first thoughts was Dead hand and Redeads from Ocarina of time, but since Dead hand was already said a few times I'll just post a pic of Redeads.

I also use to hate creatures in the water so the Eel and Fish with sunglasses that would swallow you in Mario 64 scared the hell out of me, along with the Eel in Shadow of the Colossus.

C'thun in WoW was pretty scary in a Lovecraftian type of way. Broodmother comes to mind for the buildup. Killer Croc from Arkham Asylum also has that "hunted" fear factor.

Beelzebub in Castlevania: SotN takes the prize, easily.

For a shooter, I remember some of the bosses from the Wheel of Time Game to be rather creepy. The Animated pile of corpses in the deserted city of Shadar Logoth, The Black Wind, and the mirror image you fight in the White Tower were all some rather creepy and tense moments.

Although I guess one could argue over whether they are "bosses" or not, I'd have to say the "The Beast" from Homeworld: Cataclysm. Bar none the creepiest race ever put into an RTS game. They made the Zerg seem like an army of kittens.

Even when you played as them they could still give you nightmares:

This. Although he wasn't as scary by the time I actually beat him, since I was too scared to take on that quest until I had both Satan and Helel for Armageddon (somewhere around lv 80-90). One of the scariest moments was much earlier in the game: The party was in that one split-up mode and Junpei was on low health, standing alone in a corner. The sound of rattling chains was getting closer and closer, but I stayed on that floor because it was filled with gold hands and I wasn't leaving until I got at least one of them. He then gets really skittish, and in a tone I hadn't heard out of him before, says, "Dudes... is that... Death?", before being immediately killed. Mitsuru's normally composed voice, which I was ignoring up to that point, was now the frantic soundtrack to us running and leaving his body behind. I felt like the worst field leader ever. Now, whenever I hear the chains, no matter what else I'm doing on a floor, I will book it for the staircase.

There's a book where it's implied that they chain down Berserkers to have sex with them, deity knows how they managed to stay alive before they invented chains.

Also, are you referring to GoW 1 or GoW 3/Judgment Berserkers? 'Cause GoW 1 Berserkers freaking terrified me when I first got my Xbox 360.

Easily the Ichthyosaur from Half-Life for me. He just freaks me the hell out to the point where I usually have to stop playing for a bit when I get to that chapter. I just got to his chapter in Black Mesa; I'll get to that after thinking some happy thoughts.

Yeah, the HL2 Poison Zombie is not a boss. I don't fucking care, it might as well be one.

Everything about them is just plain terrifying. The way they move, the way they sound. The amount of punishment they take. Knowing that despite all the horrible mutations and being controlled by a Poison Headcrab[footnote]Also known as the second most creepy enemy.[/footnote], the human host is still conscious and fully aware of his fate. Knowing that the Headcrab in control can throw a couple of his buddies at you.

I remember finding a cavern with ReDeads as a kid. I avoided that place like the plague.

The Forgotten One from Castlevania - Lament of Innocense. It's a giant creature, suspended by the ceiling by hooked chains, and its rotting away while still alive. Its right arm and the entirety of the lower body have fallen off, and its organs are crawling with maggots. It's the most gory boss in the game--perhaps even in the series--And it is awesome. :D

That boss gave me the creeps well after I beat it, and I didn't go to sleep until I damn well saw the sun come up.

I'm not even going to post a pic or vid of that thing. *shudder*
