The cottage behind the demon door in Fable 2 really freaked me out the first time I ran into it.

AGREED.
I fell off my bed when it happened lol.
Jumped up when that happened.

BioShock had a bunch of really creepy audio diaries and moments. Can't really think of any at the moment that haven't been mentioned yet, so yeah.

WAIT I GOT ONE! When you first encounter a Houdini splicer in Arcadia.

the note in fallout 3 ill give you a shorter version of what it said. "FLYING BANANA'S CAN'T TALK FLYING BANANA'S CAN'T TALK FLYING BANANA'S CAN'T TALK FLYING BANANA'S CAN'T TALK...(this goes on for a while) They just want you to think they can"

Fallout 3. Dunwich Borers. That guy going insane on the holotapes.

AND THEN THERE WAS THAT THING IN THE BASEMENT.

Because, he was lookin' for his dad, like you. And he needed to get something from him, like you. Just like you...
That was quite good. Even the flashbacks to the building pre-war, with all the 50s style business suits and decor, and then it changes and its full of ghouls.

I think the Keller Family Transcripts were also great, and although its not really creepy I loved the fatalistic attitute of the guy in number 5:

"I've got a mushroom cloud to walk into. Have a happy holocaust!"

Eternal Darkness.
All, or at least most of the biopsy reports, read aloud by an insanity-stricken Dr. Maximillian Roivas.

"Dear God, I HAD to do it! HAAAD to!"
"You...believe me, don't you?"
"....don't you?!"

Any scene with Claudia from Silent Hill 3.

Especially the last scene that she's alive in.

In CoD:WaW, the fourth nazi Zombie map Der Reise has 6 radios. All of them, when listened to in order, are very creepy, showing how the doctor performed his experiments that led to his own demise, as well as the demise of his daughter, not to mention it ending with a creepy evil laugh.
What do they say?

Okay, you'll crucify me, but while it may not be the scariest one I ever read, it's one of the most emotionally provoking to imagine.

The diary of Jitters from World of Warcraft. .__.
It details how he awakened the curse of the Worgen (beastmen to you non-WoWers) and fled from them across the land, eventually hiding in the barn of a man named Sven who had a happy family and farm. Because the Worgen followed him, they slaughtered Sven's entire family and he came back to find his mutilated wife and children. It simply ends with Jitters saying "I'm so tired."
Love it :D

Sander Cohens' Rabbit poem. It really creeped me out since I didn't play until I went into Posiedon's Plaza.

You try listening to this guy talking about ripping his ears off and then turning around to see a pale white spider splicer throwing a hook at you're face.

For me it will have to be SH3 in which the notes Heather reads in the hospital is written by a patient who becomes dangerously obsessed with her. Reading them you can tell this is a mad man you wouldn't Heather to run into personally (thankfully she didn't but I was a bit disappointed because I was getting curious about this guy).

Resident Evil Remake, reading a guys diary how he becomes a zombified..he even writes "Ate Terry...taste good"
Or in Resident Evil 2, the Watchman's diary where you get the Magnum.

"Itchy. Tasty."

Though personally what creeped me out in a game the most was all of Tanetane Island in Mother 3.

"There was nothing in the mailbox

Except the sound of your own scream echoing back at you."

After hearing all this Fallout 3 stuff, I'm going to go back and play it again. :p

I never got far, but this talk of the Dunwich Building, which is in itself an obvious reference to Lovecraft, tantalises me greatly!

A lot in Fallout 3 were creepy. Especially the ones about the adults separated from their kids.

sent shivers down my spine...
Yep, that's the one I thought of, as well. I was only half paying attention when I picked it up, but that thing sent a genuine chill through me. Brilliant writing and voice acting.

I'm going with pretty much every piece of narration in the Shalebridge Cradle in Thief 3. God, that level was fucked up.

I'd have to say the most creepiest journal I've read in-game was from Oblivion, during the Dark Brotherhood when you read the Traitor's journal.
Agreed. This was weirrrd.
This. And Jamie's story in the Dunwich Building in Fallout 3.
