Dead set.

I know the writer (Charlie Brooker) as more of a comedy writer, so when instead he wrote a zombie horror where EVERYONE dies a gory death, it was just a shock...
Yeah, but comedy aside, he is also a very cynical man. I can imagine him doing something like that.

EDIT: I can think of a few twists, but none in games.
And the book series I'm thinking of probably isn't read by anyone except myself, here.
Try me.

I greatly enjoyed Mass Effect's
'OMG OUR U.N. BUILDING IS ACTUALLY A GIANT DEATH PORTAL' twist.
One of the greatest "lolwut?" moments in any game I've ever played.

I hate to keep going with the obvious, but I have to say that Bioshock had the biggest twist of any game I'd ever played that I didn't see coming.

It's one of the few "young adult" series(es?) I can tolerate.
The Pendragon Adventure. Nevva Winter.
Why did you spoiler that? I have heard of that series but when I tried picking it up a while ago, I got bored with it and moved on quickly.
The end of Death Note kind of got me, though in retrospect it should not have surprised me as much as it did.
The movie Fallen had a great ending that takes everyone I know by surprise the first time they see it, which is particularly humorous considering that they say the ending at the very start of the movie and it still takes you by surprise at the end.
Game-wise, I can't think of any plots that really took me by surprise, but that has more to do with me waiting forever until I buy a game and by then most of the spoilers come out.
Oh, wait. No More Heroes. That was a weird ending that I did not expect.
Also, the plot summary I read of MGS4 took me by surprise.

SNAPE KILLS *censored*

Uh...to be honest, Terran mission 9 in Starcraft.

What the fuck, Arcturus...

And then the whole rest of the campaign, I really didn't realize until it hatched what was in the Chrysalis. Granted I was like 9 years old but gimme a break. I thought it was crazy.

From RvB; Reconstruction;

Church is the Alpha AI

I was genuienly amazed at that one.

From The Others (the series), and Cube;

Everyone dies at the end

From Sarah Connor chronicles;

Derek getting killed by a random terminator near the end of the second season

Those are the only ones I can really think of right now.
