The Citadel in Half Life 2, particularly when the place is falling apart and the equipment becomes unstable. Calling it a "madhouse" would be an understatement.

OK, being more specific:

The scene you witness in a mirror when the psychotic husband is killing one of his wives by stuff awful things down a funnel into her mouth while she is strapped to a table.

A close second is some of the methods of death from the chair with the giant swinging axe blade.

This game gives me nightmares to this day. Why did my parents let me play it when i was only 10? WTF were they thinking?
Okay so I just read about the four murders on wikipedia.. and jesus christ, I can't believe that... holy shit.
Couldn't have said it better myself. This was a really obscure game from what I can tell. It was like a snuff film though. Holy shit is right.

Man, that jack in the box in DK64 from that video scared the ever-loving crap out of me, especially since it was the first 3d game I'd owned. Seeing it now, it looks like a retarded duck, but man that thing was creepy.

What else... oh, right, Floormasters. After the first one got me, I quit the forest temple.

Right now I'm playing "The Intern's Story", a Cthulhu-based Mod for HL2. I've only gotten as far as a darkened basement so far and it's already creeping me out. It seems that the creators are doing a really good job with frightening noises and disturbing music so far.

In Half Life 2 Ep 2 when being chased by the Antlion Gaurd, my adreneline was pumping and I was scared. I guess I am easy to startle. Ravenholm wasn't too fun either in that regard, I freaking hate fast zombies.


Dear Esther looks really disturbing and creepy (in a good way) I keep putting off downloading it.

I'm torn between two - the first encounter with Alma in F.E.A.R, where the entire corridor sets ablaze; and the first time I fought a Brute on Dead Space (it finished me in style; it was quite a shocker)

The level in Call of Cthulu: DCOTE where you have to escape the hotel and barracade all the doors. You have no weapons and they will kill you!

The Gate of Pain in Guild Wars: Nightfall is freakin' weird. It looks like the cortana level from Halo 3. Actually, lets add that too.

One that stands out for me is that one from Castlevania: Aria of Sorrow on the GBA. I can't remember its name, but it's a big ball made out of bodies. Gross.

Pretty much all of American McGee's Alice, particularly Tweedle Dee and Tweedle Dum and the kids in the schoolhouse. I think this game was what made me scared of the song 'Ring Around the Rosies'. That game disturbed me ... It was like playing through my childhood melting away. Fun, though.

Nemesis from Resident Evil 3, the first time I played through this game whenever he showed up unexpectedly (which happened a lot) I would freak out and either shoot wildly or run. Then Pyramid Head from Silent Hill 2, he stalks you and rapes his victims. He doesn't show up as much as Nemesis but at Nemesis let you know he was there by yelling STARS! Pyramid Head just shows up quietly and by the time you realize it's him, it may be too late.

I know. That room would have been disturbing in anyone's mind, but Milla? That's gone from "disturbing" to "mind-fuck". Sasha's backstory is pretty messed up too.

The boss fight against Majora's Mask freaked me out when I was younger. Specifically, Majora's Incarnation (part two of the battle). The music, and the limbs, and the way it moved...it wasn't scary just..really, really creepy.

"We don't go to Ravenholm" (HL2): Not the zombies so much as those damned headcrabs always trying to eat your face off, especially the poison ones that hiss before flying at you screaming. Good God I hated this level so very much.
