Hm, mostly it's just the ambiance that gets to me, as opposed to any particular fear or phobia in game. In real life I have acrophobia (vertigo) and deep water, but in games if I get the option I go cliff diving for the thrill of it. In skating games, I LOVE half pipes, bowls, etc places where I can catch absurd amounts of air because while I Do skate, these things terrify me, but in the game I can (at least vicariously), ignore my fears and DO these things.

I don't like schools or children in games though. I really don't like children in real life, and in games they're just plain creepy, and schools? They're FULL of children. If they don't CURRENTLY have children, they have the GHOSTS of the children. Hospitals are mildly creepy, but schools are worse.

Surrealism, after playing Condemned 1, I realized this fear whenever the main character would have a "Dream Sequence-thingie"
Also the attic area of the last boss. Those damn twitching arms. o_O
Almost forgot, Silent Hill 4. Giant head. Dead Babies. I need say no more.

Any area that makes it harder and harder for you to actually get out of and get back to someplace "safe", especially when all of your supplies start running out. You know the routine: the dungeon keeps going deeper and deeper underground and then you get lost of something and it actually becomes harder to go back the way you came than to keep going forward.

The Frostcrag Spire Reborn mod for Oblivion? My worse fucking nightmare incarnate. Try going down into that city far underground in the mountain. You'll see.

I hate falls/cliffs. Whenever I'm standing on a ledge, scaling something, or even those jumps at the beginning of HL2: Episode One, my stomach drops as I do.
I can make those jumps within the first few seconds of Half-Life 2 (thank you very much), I just don't like those gaps.

As soon as I see that the flashlight has a limited battery, I start shaking.

Things that res when your back is turned. I found out Mart's Monster Mod did this to Undead the hard way.

Ceiling/Wallmasters from Zelda. You know, the hands that fall off the roof and grab you.

Agreed. Nothing as tense as a long underwater section with just enough breath to get you through.

Underwater areas were you see something big off in the distance, but your too far away from it for it to attack you. You just know you have to head in that direction and as soon as you get too close it will be on you fast. I am fine in water levels when things just come after you but preparing to go after something just sitting there unnerves me.

The worst part, this all started for my in Mario 64, that stupid eel with his head poking out of the ship, I just knew I had to go in their somehow but I swore if I got close he would eat me and swallow me whole (after the classic mario games and those giant cheep cheep fish that ate you to instant death, I didn't wanna chance it).

Those scare me too, they just creep my out, especially when Oxygen factors into it, always afraid you can't get out in time.

New enemies and the sounds they make... IN THE DARK seriously I panic if some not easily identifiable by sound comes my way in the middle of the dark. also thick grey fog and storms freak me out in games.

When the ability to move freely in the X,Y, and Z axis restricted or removed. like in Dead Space when you get grabbed by the tentacles, that just freaks me out . Also falling, the fact that i'm high does not bug me, but falling does. Curse you Mirror's Edge, even though i loved that game, but I hated it when i fell. Also atmosphere in games can freak me out. Case in point; i was playing Dead Space at like 2:30Am,( not a good idea) anyways i was kinda freaked out, and out of the corner of my eye i see some something running. I look out my open window, and my cat comes charging at me from outside. It jumps and lands on my window screen, claws out. it gets stuck on my screen for about five seconds, before running off. Needless to say i almost had a heart attack. The fact that my cat jumped on my window wasn't scary, but the mind set the game put my into made it unbelievably scary.

There's a level in one of the first tomb raider games, the second one I think, where you start in water. You know there's a shark. Of course there's a shark. Do you know where he is? No. Did he just munch your slightly-square-but-oddly-distracting ass? sure did.

Edit: the path. the whole damn thing. shat me up good and proper.

Getting jumped by a pack of Deathclaws. Every time it happens I just put down the controller and walk to the kitchen for a sandwich. There's no use in fighting anymore =(

Water. For a couple of reasons:
1. Grand Theft Auto's INSANE harbor patrol who kill you on sight.
2. Jak 3. I'd always had a fear of video game water before, but the one time I actually decided to try and swim somewhere I got eaten by a giant tentacle.
