Christ that was a difficult one, bloody scrake jumped out and tried carve me up like a christmas turkey. His chainsaw didn't do much for him, couldn't get close enough before I blew his head off with both barrels of buckshot. Oh well, no biggy, just one last monster left and..... Oh hell, a fleshpound (Or L4D tank equivalent) bastard isn't getting me, eat this! *Click**Click* Damn, pistols it is then! *Click* GAH! GRENADES! *Missing* For crying out loud!

Long story short: Running out of ammo just as you come across a boss in a game. Has happened like twice today.

When you have about 50% health left when you beat the boss and you go " Yeah!!! Im the bes... wut?" and the boss has entered his 2nd form out of 3. Epic fail every time :p

Any form of killer plant, insect, or formless organic masses. Prey actually gave me nightmares because of the "Organic ship" thing. Every time I would be barfed on by those damn ceiling things I'd fly through the roof. The flood also freaked me out, at least in the first game when they looked all moldy. For some reason they got less creepy the fleshier they got. The carrier forms still freak me out though. I would list fish monsters, but that's actually relatively rare. It's only if the fish monster is a formless organic mass, or never attacks from the front.

Anywhere where it's either completely dark or you only have something like a torch for a teeny bal of visibility.
I can tell they're gonna throw something at you, but it always makes me scream like a little girl anyway.

Darkness.
When it's so dark that I can hardly see anything.
It doesn't really scare me though, but it's really annoying.

Dying in games where you have to rely on checkpoints rather than just saving manually. And it's even worse when you're just about 3 steps from it...

sections of games where acid is rising or the floor is falling out from under me freak me out and i start getting twitchy ^__^ . same with sections where my vision is obscured. i.e. fog, as in bioshock in that bit with the dentist room where the smoke comes in and then every thing has changed in the room

I hate mental homicidal patients. Like in Max Payne when you hear the psycho's rambling or teh babbling of Splicers in Bioshock.

Snipers (Because I never use long enough range weaponry)
Things which defy all laws of death (Example: zombies in oblivion which take 50 or so hits to kill, and took of half my health in one hit. And there were seven of them.)
NPC Allies. No, wait, all Allies. (Was it really necessary to throw SIX frag grenades at that grunt I was trying to melee?)
EDIT: I forgot a few; Jarate, Flamethrowers (Guess which class I play on TF2?) and on Dawn of War 2, seeing THREE Carnifexes charging my base at once, supported by innumabrle gaunts. Oh, and this is while all my heavy support and elite are assaulting the enemy base. Damn you wraithlords for not walking faster!

all of a sudden sounds, or even worse when you're going through a part which has no sound, think they're gonna throw something at you, but they dont. Or in TES:2: Daggerfall when walking through daggerfall at night and at 2 am irl with the light out, and all of the sudden you hear him: vengeance! *shudders*

Any sort of horror game or horror FPS. You think everything's fine when suddenly, Red screen.

This is especially in AvP.

Nothing, or at least I thought so until yesterday...
I downloaded the mod "Radio Existence" for Fallout 3. I turned it on inside the metro.
I nearly pissed my pants after 5 minutes.

Would it make you feel better if you knew it was just Yhatzee whispering "father no!" in a high pitched voice?

As for my in-game fear... Eurgh, just, Silent Hill 2. It is the only horror game that's ever had an effect on me (except Alien Trilogy, but I was about 7).

Oh, and Ghouls in Fallout 3, and Molerats... I'm guessing I just don't like the whole "I can hear it coming but I can't see it yet" thing.

Sneak-attacks and stealth based units in RTSs. I have a habit of always walking into those.
Oh, nobody in the nightelf base and the narrow passage by the river there is clear? Cool, I'll just send my hero across and BLOODY MOON PRIESTESS ***** WHERE DID YOU COME FROM?!

And small, flying enemies in horror shooters.
