Any enemy in any rpg that is pure and solid rng. Like:
They're charging their special attack and whoops they just happend to all target your healer and whoops look at that your healer got one shotted press x to reload. That is not difficulty that's just pure luck.

The kind that can do devastating damage and keeps chasing you no matter how you try to escape. This includes Creepers from Minecraft, any creature from Fallout New Vegas and Fallout 3, and Tanks in Left 4 Dead.

And then there's f#$%ing Talon Company.

They aren't hard, but jockeys in Left 4 Dead. They bounce around, have that creepy laugh and are a pain in the butt to shoot. Then they jump on your face and do unspeakable things. D;

Hate...oh the hate...

Cazadors from New Vegas. Not so much for killing ME all the time, but for killing my companions pretty easily on Hardcore.

Also Praetorians and Scions from Mass Effect 2. Oh, and Husks with the Infiltrator. Actually, everything from the Collector missions.

Any enemy in the first Prince of Persia remake. Awesome platforming - tedious and lenghty fights.

A bunch of things, but I can't remember most.

So I'll say the robots of any sorts from System Shock 2 as a pure PSI character (no, game, pyro/cryokinesis does not get the job done), especially protocol droids. Imagine something as friendly and helpful as the Portal turrets. They walk up to you. Then explode in spectacular fashion for ridiculous amounts of damage. Fun times.

Tenshi Hinanai and Reisen played by the computer, in Hisoutensoku. Highest difficulty. Reisen is irritating, while Tenshi somehow messes up everything I do. The laser spam doesn't help any. Komachi is fairly awful too, when played by my sister.

In Age of Wonders 2, halflings. When played? Worthless. When played by the cheating cheater of an AI that cheats? Makes the zerg look jealous.

Also, for all the XCOM players out there: Chryssalids.

I'm surprised no one has mentioned these:

I don't really hate fighting them - but in Call of Pripyat, more than one can become extremelly annoying. They rarely attack you head on, always attempting to attack your flanks or rear. It's a PITA to deal with one attacking on your left side, and another attacking on your right . . .

Pregnants from the Dead Space series. I can never do it right. And when you can't do it right, then you bring out the little buggers that take even more ammo to kill. More precious ammo.
Shoot the legs out first that makes it a lot easier, then when they are on the deck take out the arms. Never shoot them in the belly

Black Necromorphs, that boss from DOA4, Metroid Prime, that Necromorph you can only slow down and not kill, Deathclaws(NV), Cazadors(NV), Security Holograms(NVDLC), Scions, Harbinger(on insanity anyway).

The worst is this NPC in runescape that you have to beat by yourself, was the master of a minigame, was actually a factor in me quitting that game.(above level 600)

Any big guy that just blocks all the time

his shield or defensive whatever might as well just be a big sign that says "I'M HERE TO WASTE YOUR TIME AND LENGTHEN THE GAME LULZ"

In any hack-and-slash. Any enemy that thinks blocking 99% of the time is supposed to improve the difficulty of the game just pisses me off really!
What I was going to say ;)

EDIT: Oh yeah, and the fat dude from Dead space 1 and 2. You the ones that if you shoot them in the stomach they release those small enemies that drill into your back... and the Stalker from Dead Space 1 and 2.... and the Puker from Dead Space 2... alright I hate a lot of enemies in Dead Space... including the Lurkers... from Dead Space 1 and 2

.... and the baby bombs... from Dead Space 2

EDIT EDIT: I cant believe I completely forgot about the Creepers from Minecraft. I dont know how many times they ruined my masterpieces, so I turned the game down on the peaceful difficulty. They pissed me of so damn much

And anything that has a knockback area effect that stuns and then spams the move....
Also trolls in single player guild missions on White Knight Chronicles :/

Cazadors, super mutants, and deathclaws in New Vegas. I eventually got better, got a fresh anti-materiel rifle and kicked all of their asses (including the legendary deathclaw/cazador), though I still hate them and they're still annoying.

Feral Ghoul Reavers in Fallout 3. Even with the dart gun, they're still fast, insanely accurate with whatever they're tossing from their chests, can dish out melee damage like a dearhclaw and can withstand so much punishment. Oh, and there's a bunch of them in the tramway when completing Broken Steel.

Wind Riders in Guild Wars. They constantly interrupt/damage everyone, drain health, shatter enchantments and deny adrenaline for melee weapon users. Oh, and they come in swarms. Also from Guild Wars, Awakened Cavaliers. They have two ways of resurrecting their buddies, are in huge mobs, and in those mobs there are at least two of them with the addition of a monk capable of resurrecting. Luckily, said reviving can be prevented.

Ultima Weapon Final fantasy VII

"Hi, im here to attack you with stupidly powerful magic, knock seven bells out of you, then fuck off leaving you with no XP, loot or money, then make you FUCKING CHASE ME, until I arbitarilly stop somewhere for no reason just to make you do the whole fucking thing again"

Not particularly difficult, just used to piss me off
