ME: Could not kill the Rachni Queen. Something about having an intelligent creature in captivity with the means of killing it being Acid ready to pour into the 'cage' Maybe it would have been the right thing to do but it also just seemed horrible.
Dragon Age: Origins: I have never killed Connor, it seemed pointless when there is the option for the circle to help, And since it is stated something like you can make it back to the Circle in a day or so, it seemed kinda silly to kill the child and possibly piss off Eamon completely if he got up. (This one is just to add though, from a roleplaying perspective I think the circle is the best way to go. if it was a better choice, I would kill him in a heartbeat.)
The Witcher: I never side with either side, both have serious flaws in their methods and it was kinda annoying that they make you choose one at the bank robbery, luckily you can tell them to fuck off and go your own way a couple of chapters down.
There's quite a bit more probably but I can't think right now.

I came into this thread thinking of that actually, though I always either gave her the money or just told her to go away. I tend not to do full black or white playthroughs so it wasn't a problem, but yeah, It's been a long while since this but if I remember correctly when I played, Her voice acting sold it.

I am not sure I follow here? You're talking about killing the Dog in Dragon Age: Origins? How do you not have a choice as a Noble? I've done 3 playthroughs as a Noble and one as a Mage, non of them required me to kill the Dog.
