I have died many times in Skyrim by completely dismissing the Forsworn. They're pretty damn tough for a bunch of guys wielding sticks and wearing loincloths.

Don't feel bad, I've done that.

Mine has to be in Skyrim, just got out of a huge battle in a fort involving around twenty bandits and no less that two dragons that decided to join in. Got some awesome loot, levelled up, didn't bother saving and headed off into the wild foothills around Rorikstead.
Promtly ambushed and slaughtered by a pair of ice mages.

Yeah, haven't played much skyrim since then...

My most recent I can remember without digging up some dumbass memories, was recently in Darksouls that I seem to never learn.. I mean EVER. I always do this, and It is a reason why I HATE HATE HATE narrow shit. In Anor Lando I am fighting those ghost guys on the pathways above the chandelier and for some stupid reason I need to roll for dodging. HATE narrow path crap.

Falling off The Great Lift in Thousand Needles (I think, I haven't played since Cata, and didn't play religiously). To add salt to the wound, I was in the process of telling my friend to be careful NOT to fall off.

As for a stupid death I caused (Not to mention some issues recovering the body), Some random person paid me 1000 gold to get a ride in my chopper. I drove around Dalaran and into the sewers and drove out of the pipe underneath Dalaran. The bike landed on one of the floating platforms outside and back than you didn't get a "Spectral Gryphon" to fly. His body was stuck in the clouds and his ghost was grounded outside of Dalaran, AND he was 1000g lighter.

Fought Gwyn with my Dual Bonewheelshield guy. Suddenly GFWL had a hickup and i got disconnected and thrown back to the start menu. When i reload it sets me outside the fog door with zero health left and i die.

If we're espousing our Wow deaths first, mine goes to a random botter i met. Tailed him around for a while until he stopped on top of a cliff, waited 'til he'd finished mining the node then, as he'd just finished casting him mount, i piffed my axe at him. Watching his bot march him straight off a cliff was only topped by the sight of his void walker minion "Wait for meeeeee"ing it off the cliff after him.

As for myself; trying to drop a building on someone in Battlfield 3 and getting killed by the external stairs as it came down. Missed the kill too.

I felt several of the "deaths" in Final Fantasy 2 (well American 2, really 5? I think 5) were pretty stupid. The entire game consisted of leveling up a character, only to have them "jump on a grenade" in a cutscene and then you lose them to be replaced with a different character you have NOT been playing the last several hours with. And it kept happening over and over... it started to feel a little forced.

As for my own deaths... a lot of dying in Minecraft is due to stupidity. I remember venturing a long way from my home to find cows (I needed leather) and finding some but darkness was approaching. So I constructed a temporary dirt structure to hide in in a hillside. Decided to pass the time digging into a hillside (in extreme edge) and wound up getting mobbed by silverfish for my trouble. Barely survived with one heart left, only to starve to death seconds later within arms reach of about 5 cows.

The worst is in Tecmo's Deception series though. One of the unlockable weapons is the dreaded "banana peel." Slipping on one only to get thrown into a pit of pit of oil, then lit on fire by an arrow trap, then catapulted across the room into an iron maiden, which released you into a cloud of confuse gas, making you stumble into a boulder sized snowball that rolls you into a lava trap... well it goes on from there. It's worse because I put the banana peel there in the first place.

Oh and "banana..." lots of times my team mates looking at me after a Super Banana Bomb eliminates 50 - 75 percent of my own worms and team and only damages one opponent worm.

Well dying to your own jungle in LoL is embarassing. Fucking Rengar's first clear is hairy man... It's a non-issue once you get Madred's first back but i've had to full on run the hell away from Blue Buff on 50HP. It's a freaking boss fight at that point. Any other jungler can take full clear easily it's just Rengar. God forbid if I go Rengar vs Shaco...

Oh and misclicking random moves in BlazBlue makes me look fucking stupid. Doing a perfectly fine Noel combo then I get a random as hell heavy Optic Barrel targeting the other side of the damn screen. He just drops out and kills me. Also my entire experience with Tsubaki is AHHH NO, WHAT?! WRONG MOVE TSUBAKI! I swear her charge move is Vergil levels of unsafe. And her crouching heavy is a damn mini antiair kick thing that whiffs horribly 90% of the time. Ehh maybe i'm not managing Install properly, i'm used to finding openings to charge things by now. I mained Phoenix Wright in UMVC3 after all.

It's safe to say I play Tsubaki solely for her badass wing aesthetic and awesome stage music. I mean the same could be said for Makoto and recently Hazama but I can actually kind of play Makoto. Hazama I just play for his hilarious soundclips

Every once in a while I manage to somehow miss the haystack when preforming a leap of faith in an AC game. It makes me feel like an idiot every time.

There wasn't enough lock outs in ME3. There should have been a plethora of situations that could only arise if you ticked specific boxes in the first and second game. Fuck the people who just started - they can get the default experience (which is still more than enough) and if they really want to they can go back and make their own story with their own outcomes. It's not like Mass Effect was some indie series that no one knew about - if you didn't play the Mass Effect series until the third game came along you shouldn't expect to have a customised playthrough, its your own fault.

That is a part of the reason most long time Mass Effect fans were disgruntled with the last game. The choices you made meant almost nothing at the end because the developers were too concerned with making it generic so everyone could play it.

I was doing some jolly co-oping in Dark Souls last weekend, helping another player fight off the Gaping Dragon in the Depths. All was going well, tail removed and things under control when I took one step too far and slipped off the ledge to my death. I was so focused on the boss that I wasn't paying attention to where I was walking. That was a total derp moment and embarrassing as hell haha. :p

Far Cry 3, just after Citra tells me to make sure I come back. I run straight for the edge of the pillar we were on and jump into the ocean ...

I had some many stupid deaths..
But a resent one is real case of D'oh!

I was hunting Vista's in Guild Wars 2, for people who don't know what Vista's are, they are a little cinematic camera swoop around the environment as a reward for solving a (sometimes rather complex) jumping puzzle.
Needless to say these vista's are on rather high locations. On a top of a mountain, edge of a cliff, etc.

After spending nearly an hour on trying to get to one, refusing to give up and look on youtube, I finally got it. I was on the same platform, just needed to take one more step.
Until I accidental hit the Walk-Forward button twice.
This makes your character do a little forward dodge roll. Right off of the cliff.

Smack, dead.

i seem to remember the cause of her remaining "in a continous mortive state" (dead is politically incorrect Guys 'n gals :/) is her being "Dead, DEAD" not just KO dead. But i do not remember cloud throwing down feathers at her, is this talked about later or?

No, I've jumped off the rises in Shattrath, I've fallen from Dalaran, I've leaped off of Wyrmrest Tower, dismounted above Stormwind (I'm a warlock, I was trying to see if I could save myself by using the teleport they got - It rarely worked as planned), fallen off the Darnassus Tree of Life, and fallen in the lava in Ironforge.

EDIT: Fun anecdote, actually. I was level 80 when I fell in the lava in Ironforge, so I actually had enough health to spend ten minutes assembling a group with fellow guild members, one of whom was another warlock, and have them summon me out of the lava to Dalaran.
QFT. The Aldor rise in Shattrath was just cruel. That elevator claimed so many lives.

I especially love the part where you click your flying mount and proceed to jump off a cliff.

...Only you mis-clicked, and it's your ground mount. It's a very Wile-E-Coyote moment as you plunge to the ground.
It's even more embarassing if you are a mage with Slow Fall, haha.

Apart from that, Uncharted 2. Oops, jumped of the building - death and dramatic music. Fell of a ledge - death and dramatic music. A drop of two metres - death and dramatic music.
It just felt stupid after 10th time I died in the same half an hour. This never happened to me in Assassins Creed *grumble*

In skyrim not a half hour ago I was wandering around the forgotten vale and happened upon a frost troll (I'd forgotten about those fuckers, it's been so long since my first playthrough of Dawnguard).

It stomped once.

I immediately died. I had full health. I was level 65.
Seriously even for a mage-build that was just awful.

Paying planetside 2 flying around in my mosquito decide my ammo low and its time to reload

Usually, I fall for the classic "getting caught under a rapidly descending elevator" routine. It doesn't happen in games these days, and I kind of miss it.

In LA Noire there is that getting tagged by a car that is moving at less than walking speed, only to instantly die. Some of the deaths look especially ridiculous in that game - like when you stand still in the tar pit for too long, Cole Phelps submerges straight down without the slightest look of concern on his face.
