If you're playing the PC version, enable noclip (type "tcl" without the quotes on the console) then explode someone. Results are freaky to say the least.

two real scary glitches happened to me. one was cool, it was when i was in the middle of fucking nowhere and i turn around just in time to see a super mutant, spawn and fall 50 feet to the ground then die.
the second not so cool, when i got the E-MIRV, and only 2 of the eight nukes would explode at a time, the when i thought it was safe i moved to where the nukes had left a crater, only to have the other 6 explode right under me....lag=uncool

I've played fallout and a yao guai falling...falling...slowly than bang in the ground! but accept that mabey some laggs but not more than that...

you will see the glitch i am talking about clearly though. Wierd-in this video it disapears. It doesn't in the other guy's com.

For example, recently I started a new character, I was just heading back from super-duper mart with the supllies for Moira, when I was met by two Enclave soldiers, which aren't meant to appear untill you have reached a certain point in the main quest.
They slaughtered me.
Another odd thing I once saw was a group of super-mutants fighting eachother, I just stood back and watched, then sneakily shot the last one standing.

I was wandering the wastes when I came across a Deathclaw fighting a Albino Giant Scorpion and three Giant Scorpions and a Sentry Bot. The Albino Scorpion won.

I haven't had any major mind fucks. I play it on a PS3 (cause my xbox broke, lame), and the only real problem i have ever had was my sound cuts out randomly.
Oh, the first super behemoth i ran into (the one outside of the Radio building) was stuck under a low hanging roof, so i thought it would be funny to beat him to death with my fists.

I can't remember which vault it is, but there's an abandoned vault you can go in where your character starts to go crazy, I guess. In different rooms, all the colors turn into shades of blue and look as clean as vault 101 in the beginning of the game. And then there's the computer terminals that say a bunch of random things and then stop working. I couldn't figure out if there was any point to that vault. But it definitely screwed with your head.
If you know what I'm talking about, does that vault have any significance or is it just meant to make your character temporarily insane?

The only major mindfucks I've come across would be having a Deathclaw float away after killing him outside the Dunwich building, and on my first playthrough, when I went to the Super Duper Mart, a Chinese commando from the Tranquility Lane simulation dropped out of the sky, giving me a perfect Chinese Assault Rifle and Chinese Commando uniform.
Don't know if this was scripted but as i was walking from Super-Duper mart after casing the joint for Moira I die in a fiery explosion.
When it reloads the autosave on the way back i notice what killed me, one of those spooky crying explody death prams surrounded by frag mines.
The mind boggling part is that all the mines were not armed when i tried to disarm them and the explody death pram was not 'working' as such, it just had the appearance and i could walk through it and no interactions available.

In a bad install glitch, I stepped on a rock and suddenly was a mile or so above my step. I fell for a while and died.
