I remember the Shoggoth from reading At the Mountains of Madness...it sounds fucked up.

Never got to play that Cthulhu game, though...I am so disappointed in myself.
I couldn't find it anywhere but GameFly, that netflix-esque rental service. I just got a free trial, rented it, beat it, and returned it. It's really got no real replay value, so I suggest that method.

Stalkers from Half-Life 2. Yeesh. Those things scare the shit out of me, despite being mostly harmless.

Oh yes. The Centaurs or whatever they were called. Yeah, they're quite disturbing, especially since, like the stalkers, they used to be human.

There were those big demons in Doom 3 expansion that had CRT monitors attached to their face with images of normal human teeth on them.

Zelda OoT had those grave monsters that dove in and out of the ground with the hands that grappled you.

Big Daddy/Little Sister combo is always sweet.

Anything from Morrowind. While not really "f'd up," they were refreshingly different to the point that everything you fought seemed new and original like any other "f'd up" enemy. I'll always love Nix-hounds for that reason.

I would say Nemesis from Resident Evil. Between the menacing voice and the fact that he's twice as tall as any of the playable characters, that guy gave me the willies.

Not really an enemy per se, but The Darkness had a sequence where you go through a hellish vision of WW1. There were zombie germans there, that were pretty messed up, but more creepy for me were the NPCs. There were weird patchwork British soldiers, sewn together from bits of other men. One guy in a trench would shoot himself in the head, only to come back to life and do it all again. A basically skeletal corpse sat in a sewer, playing a harmonica. A field hospital where men with no limbs, no eyes, no faces etc. sit there begging for their mothers. You talk to a man with no limbs and all his face blown away but his mouth, and he says something about 'keep a stiff upper lip' before sobbing and calling for a nurse, or his mother, or anyone. And when you went inside a massive railway gun to fire it, there was this one creepy German dude sitting behind you. You walk in, and all you hear is this deep, menacing voice. You turn around, and there's a limbless zombie German talking to you. Creepy stuff. Damn, that entire part of the game was creepy as all hell.

Chalk me up for the Regenerators from RE4. In all my years of gaming, that was the first enemy that really, genuinely, frightened me. I missed the infra-red scope, so anytime I came across them all I could do was empty all my ammo into them and hope it was enough.

Everytime I heard their breathing it wasn't sort of "*chuckle* heh heh, oh shit huh?". It was blind, manic, "switch this damn machine off" terror.
