Poor guy... how he managed to get a job there is beyond me. His presence did kind of make LaCroix look slightly foolish tbh. In fact how he even became Prince is a fucking mystery.
For me its gotta be this:
Gremio's sacrafice. The guy literally locks himself in a room filled with man-eating spores that eat him alive while hes talking to you through the door. I mean its cool you can resurect him towards the end of the game but at the time its pretty upsetting!
If I have to choose right this minute, then the ones that I always think about when people bring this subject up is actually the deaths in Goodnight Mister Tom.
The deaths of Will's little sister and his best friend Zach to be exact.

Something more recent would be... Nope, can't think of anything. There was that film I saw a about 7-8 years ago, where a mother of 8 (I think) finds out she has cancer and, knowing her husband wouldn't be able to take care of the kids, she sets out to get them good families before she dies. My mother died of cancer very suddenly when I was 7 so it hit close to home. Dunno if that one counts though.

I didn't kill the technician (i did find the body of the real one but figured it was a bug)
so therefore on the final mission the pilot blew up
it just came out of nowhere, and knowing that i could have prevented it made it even worse
godammit I miss him
Xenoblade chronicles, and what happens when you meet/fight Metal Face for the first time. Sadness so severe, I had to stop playing, and take a day of mourning.
Damn, I liked Tali, I didn't really know what I was doing, thought it meant that it would eliminate a small squad of the quarian fleet that was too close, not the lot. Then she threw herself off that cliff. I tried to grab her but I was too late.
I thought about downloading a save or savegame editor or something but I thought that it was too powerful to simply edit out. Mordin's also hit really hard but not in the same way, he died a fucking hero. And that's saying something coming from me.
I'd have to agree with people on the Code Geass and Mass Effect choices, though I believe...
... killing Mordin is a great scene for the extreme renegade Shephard. Kind of a moral event horizon, and really cements their willingness to do anything to complete their mission. Those people who refuse to do it even on alternate renegade saves are missing out on a lot, in my opinion.
I've heard a lot of criticism of the story in this game for some reason, but I really enjoyed it. Joker's entirely avoidable and self-wrought demise managed to seriously affect me, which is strange considering how clearly evil he is.

I would have to say Wash's death in Serenity. I loved Wash's character and when he had just "landed" Serenity and everyone was alive and well when they all should have died... to just so suddenly be killed. Well... here:

Your second spoiler surprised me, but also let me know that I was reading something very new and different. Your second spoiler? Well, it is hands down one of the most gutting moments I've ever experienced in all my years of reading. I've never really forgiven Martin for it.

I didn't renegade and it was still sad. All of my mates deaths left me sad. It was/is the hardest game I've ever played on a mental/emotional standpoint. It really drains.
Another really stands out for me. And as they say: you never forget your first, which for me was:

John Marston. I did not see it coming. That game was untarnished by the internet for me so it was 100% fresh, especially since I had played its spiritual ancestor back on PS2 I had actively avoided reading anything about Redemption.

at least that's how ME3's renegade shepard comes across to me, as if he/she was desperate to stop the reapers that he/she would do anything
granted, killing mordin and preventing him of getting his redemption is the videogame equivalent of drowning kittens with your own hands, but still =P

Hollis Mason was killed by the Knot-Top gang. It was so tragic, but you really couldn't help feeling happy for him, in a weird way, it seems the best way someone like that could have died
