I'd have to say:
This one. Definitely.
It's not just because of how well set up the entire scene is, or because of how great Ryan's voice actor is, or even the haunting music, but the revelation that all of your actions up to that point have been completely involuntary on your part. The fact that he demands you to beat him to death on his own free will is just as powerful, and the ultimate depiction of the character's willpower.
It's like having your mind beaten with a golf club.
This would be my first pick, it's not sad but it's one hell of a gut-punch, considering the game's been hyping up the confrontation for the entirety of the game up to that point, you're expecting a boss battle...and then you get something else entirely.

One I've not seen mentioned:

Eddie. Other deaths afterwards might have been more affecting, but Eddie was the first of the ka-tet to die, and it signalled the beginning of the group's destruction, after five books of being near-inseparable.

Oh, and as a kid
Littlefoot's mother.

All of these have been said before but

Death Note
L

Fullmetal Alchemist
Hughes (it was worse in the first imo because they built up his character more, or maybe I just wasn't expecting it first)

Game Of Thrones
Ned Stark, still one of the most unexpected deaths ever

Not sure if it counts but Bioshock
When you kill Andrew Ryan with your own hands.

In Valkyria Chronicles
Isara and Selvaria's deaths <youtube=WzancAU4cRQ> <youtube=VZ_Fr2m7HGc>
In Ace Combat 5
The way Chopper died because he didn't want to risk civilian lives
In Ace Combat Zero
PJ's death, and the way he accidentally saves you by taking the laser beam Pixy fires at you
In Spec Ops: The Line
Lugo, and the way he got hanged by the "civilians"... I luckily had an M249 SAW to make sure they all paid the price. Bastards.
Persona 3 and 4
Persona 3 had Chidori, Shinjiro and the Main Character. Persona 4 had Nanako
And lastly Clannad: After Story. (which I have been ninja'd with a couple of times)

Oh god where to begin (I'm terrible with sad moments in games/films and tv). Basically in every one of these, I cried or was brought close to tears.

Joker's death. I love Mark Hamill's Joker to pieces and I will happily argue my case for him being the best representation of the Joker ever (srsly, I swear Heath Ledger is only popular because he died) Honestly, I thought he was faking right until the credits were done and I was booted back to the main menu. Then I just sat there and had a little cry. It was such a hugely emotional moment, and playing through the DLC just brings it home even more because Batman is still obviously deeply effected by it.

He gets his own little section, obviously. Ned Stark got the most unjust and undeserved of deaths, and I for one will throw a little party when Joffrey gets the horrific death he has long deserved. Boromir's death in LOTR also gets me, because even though he was tempted by the ring, he still gave his all to save Merry and Pippin. It also makes it worse when you've read the books, and you know that he's only there because Denethor didn't want Faramir being in the Fellowship.

Mordin gets the obvious mention, but also Tali. I chose to save the Geth over the Quarians, and I honestly felt like the most horrible person alive.

Oh god this film... when the Cajun man dies is just all kinds of tragic, but when John Coffey dies right at the end, and he tells the prison guards not to put the black hood over his head "because he's scared of the dark"... I was just a blubbering mess for the rest of the film. Goddamn you Stephen King.

RIP James T Kirk, you died in the most unfitting way possible.

Let's just say I can't listen to Bob Marley without thinking about the dog...

When Fred, Lupin and Tonks die. I don't think they made nearly enough of a big deal about this in the film as Rowling did in the book.

That. I am sad for every ingame animal friend that dies. I even tried to save all my ant lions in HL². Frankly didn't work, but. They were so cute.

Oh, one I just remembered:

When Sherlock "dies". My and my friends affectionately call this the "feel ALL the feelings" episode, because you simultaneously feel anger, sadness, confusion and doubt. I think it's more Watson's reaction that makes me blub, though. "Please don't be dead.." :(

From the Song of Ice and Fire series

The entire Red Wedding.
That was gonna be one of mine. SoIaF has more than a few of those moments. I'm gonna put up
Ygritte. From the moment they scaled the Wall it was only going to end that way, but it was written in such a way that brought a lump to my throat.
It'll also be interesting to see what happens with
Jon after he was betrayed by the Night's Watch
as it has clearly been written to leave a question mark over whether that person survived or not.

Moving away from SoIaF and into The Dark Tower for a moment, I was strangely touched by
Oy's death. Roland's treatment of him beforehand, Oy's foreknowledge of what is to come, Roland's realization of same - it's just got me. What's more, it got me as I was sat at my desk in a busy office at lunchtime. Not a good look.

The 80's animated movie pretty much killed off my entire toy collection, but seeing how no-one ever died in the tv show it was a huge shock when Prime died

Spock's death in this one. Hugely emotional death scene that even Shatner was able to act in

Wash's death. I think it's because you don't even have time to acknowledge that it's happened. There's the shock and then they have to leave other they'll all be killed

1) Code Geass - Shirley's death, and also Euphemia's death. Both felt unnecessary and left with me a with a horrible sinking feeling of "oh god no...but why? WHY??". The sadness of it all was overwhelming and left me trembling in my chair :(

2) Harry Potter and the Order Of The Phoenix (The book obviously, the movies can go screw themselves) - Sirius Black's death. Also Dumbledore's death while we're at it.

3) Naruto - Jiraiya's death. I shouldn't even need to explain this one. He was one of those badass loveable characters and I honestly didn't see his death coming, really hit me hard. And I was reading the manga FYI.

4) The Iron Giant (1999) - when the Iron Giant had assumed Hogarth was dead (he wasn't, but the giant's reaction still brought me to tears). Also when the giant sacrified himself and flew into the nuke.

Probably

Jiraiya's death in Naruto, just because I absolutely did not see it coming.
T_T i picked a random spoiler on the 1st page... :( and i just spoiled it for myself XD

OT:
Tengen Toppa Gurren Lagann, especially when Kittan, Kamina, and Nia died. Kittan and Kamina were awesome, and I had thought that Nia and Simon would live happily ever after and then Nia faded away leaving Simon on his lonesome forever... i found it really sad :(

This will be the strangest but... South Park.

When Kenny dies in the Kenny Dies episode. Sure he has died in every single episode except the Xmas special up till then, but in this one people saw it coming for once. His friends being sad, all that raw emotion, gets me every time.

And Futurama, technically.

When Seymour is sitting outside the pizza parlour waiting for Fry to come home.

Yeah, the only reason I was evil in Fable 2 was because people kept kicking my dog, at which point I'd have to murder entire towns to teach those yokels that that's bad manners.

Game of Thrones
The death of Robb Stark and his men during the feast with the Freys. It also shocked me how indifferent and neutral the writer is towards his characters.

Also, I was watching The Last King of Scotland with a friend and that movie got to her to the point where she had to go and throw up.
