I'm pritty sure it was GTA Vice City. Not my copy though. While all the other kids where shot gunning hookers in the face the worst violence I got was Luigi sucking up ghosts. I wasn't allowed an 18 game of my own until I was about 14 or so.

While not the first "violent" game I had played, Manhunt was the first game that actually caused some discomfort. That was just one disturbing game. Definitely not for children . . .

I remember how my mum got pissed off at me playing such a violent game and broke it. I was SO pissed.

Baldur's Gate. Not too gory, but people can explode into bouncy meat chunks.

Anyone Remember Chiller?
That's probably it for me.
It was a really sick light gun game from the 80s. They put it on the NES too; they should remake for the wii.
Each level was a view of a different room of a haunted mansion some targets were moving ghosts and bats, some of the more static targets were often torture victims tied to torture/execution devises. You could shoot the flesh off the victims but most of it was shooting the trigger on the guillotine or the crank on the thumbscrews.
Sick stuff.
