walking into a safe room in Left 4 Dead 2, being one of two remaining survivors, to find a witch spawned in there. she proceeded to kill us both, and i then followed with breaking an xbox controller :S

On a more serious note, I played a PERFECT run on Dead Rising. I went into the extra mode. I had the mini chain-saws, the books, machine guns, everyhting I needed. I breezed through the extra mode and made it my bi... fallacious-woman. Anyway, I get into the tunnel, I open the first grate, the game freezes. I sit still, waiting a few minutes. Yup, frozen. I go to the xbox. Turn it on and off. I start up my game. It deleted my save. My save was gone. Those 49 people I saved? Gone. Those pieces of the machine I collected? Gone. Those ten fucking bugs? GONE!
I have yet to play the game since.

On Splinter Cell Pandora Tomorrow, I got into this room where you talk to this guy you were supposed to rescue. Except when I talked to him I didn't realize there was another guy in the room... As soon as I finish talking BAM the guy I rescued gets capped! reload BAM! reload BAM! reload BAM! fuuuuuuuuuuuuuuuck!
I was playing Dead Rising 2 earlier, facing the final boss. He was giving me a bit of trouble as I didn't do much damage to him(damn the game for only giving me two attack upgrades). After a few tries, I finally had him on the ropes. I picked up some orange juice to heal, but right when I did a very strange glitch happened. The game wouldn't let me drink the orange juice, hell it wouldn't even let me equip something else. It wouldn't let me jump, or do anything but walk around, it's like all of the buttons locked up on me. Resetting the game fixed this, but I was pretty pissed because the boss was almost dead. Personally I blame the orange juice. Curse you orange juice....CURSE YOUUUUUUUUUU!

edit: if it's specifically a video game moment, it's all of the 1st S.T.A.L.K.E.R. it was just "O.O'' FUUUUUUUUUUUUUUUUUUU-AAAAAAAAAAAAAAAAAAAAAAAAAAAHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHH!"

first game i played was pokemon blue version, i had everything i needed to finally beat the elite four. after what seemed like a year of beating that game on my game boy. then my friend started a new game while i wasn't looking. and wrote over my progress. fuck
Probably Halo 3 going for the Annual achievement (anyone dies you restart to last checkpoint, 4 player co-op). As we reach the last part of the level we enter the cutscene...only for someone to somehow die IN THE CUTSCENE and then leave the game afterwards. To get to that part usually takes about an hour. That was our 5th attempt.
In Minecraft, after exploring a cave for ages with barely any health due to several skeleton attacks, I run into a zombie spawner and get sucker punched by a zombie and die, then all of my items (including a bunch of diamonds) fall into a lava stream.

He was over and playing Reach online ((a game i personally hate)).
He was trying to complete all of the dayly challanges.
His last one was "Get 150 assists"
he ended up getting sniper and swat 15 times in a row as his game modes, and when he hit 148 assists, the server resets. ((Every morning at 6 am my time))
There got like, 4 hours of grinding.

Getting home from a long day at work, all i wanted to do was play a game turned on my ps3 to get that evil yellow light then remembering the warranty ended a month ago.............fuck.
