Glitches and bugs that can be abused online piss me off to no end. I'll post some more specific examples when I can think of them.

Ultima IX: Ascension was released with numerous plot breaking bugs that made it impossible to move past certain points in the game. It was possible to trigger these bugs without realizing it, often 10-15 hours before it would become apparent that the game was irreparably broken and needed to be restarted.

A patch was later released that fixed the most egregious errors, while introducing a host of new bugs that broke the plot in entirely new places.

MW2 Care package glitch: AKA How to level up a Stinger and Cold Blooded in three easy matches. Best one is getting both a Harrier and a Pavelow with one missile. Still yet to shoot down a Stealth Bomber with an RPG, like they did in the Balkans...

Not so much a bug but an inexcusable design incompatibility:
Thief 3: Deadly Shadows on Vista. The save games are scripted to be placed in a folder that Windows moved for Vista, but the game isn't smart enough to go and find the new place like most games are. Instead, it insists on saving in one place and one place only. What is more retarded is that it will only go to that one hypothetical folder to load games too. The end result of this is that you can't actually save in the game and therefore can't really make any progress. It's blood-boilingly annoying because I've been waiting to play that game since 2001 when I last played Thief 2.

Also, this weird graphical glitch with Beyond Good and Evil that made me stop playing it where in outdoors environments these strange colours would flash all around like a strobelight or under a disco-ball. It was giving me epilepsy, so I gave up.

Uh, oh yeah, I never played past the first few levels in Half Life 2 because of that rather well-documented thing where because the default field of vision is either narrower or wider than most FPSs it can give some people nausea. I got it really badly, felt like throwing up while playing it. I know its fixable, but I haven't bothered to go back to it yet.

Half Life 2: Episode 2 had a few instances in one elevator where I ended up being stuck, unable to move, and had to re-start the game.

OT: In FO3, in The Pitt, I can't get to some of the Ingots because, when I cross the roof they are on, it turns to nothing and I fall through to my sad death.
Very common glitch, caused by conflicting files between The Pitt and Point Lookout.

I'll bet that the roof is to the left of several dome-type things, and you fall down into an empty warehouse thing. Turn your console off and on again/reload a save. It should have come back. Just do that every time it breaks, which is all the time. Piss irritating glitch, but avoidable.

Fable 2: When the monk in Oakvale was talking, I walked away, and then couldn't progress at all. It took Lionhead 5 months (although they said it would only be a week from the release date) to release a patch for this.

D2 LoD: Sometimes on battle.net I would have a low level character get rushed, and then the rusher would leave halfway through, and sometimes midway through a quest where you need quest items, like in Act II. It was pretty tough sometimes finding someone to help you finish that if you didn't have a ton of high-level friends.

I refuse to touch MW2 until all the glitches have been patched. First the javelin glitch and now the care package one. It just shows how Infinity Ward rushed it onto the shelves without thoroughly testing it. It's also really hard to find a Free For All without some idiots nuke boosting.

In Fable 2, i chose the ending
in which you get your dog and family back
And when i got to the demon door house in oakfield where i live, there where 2 people. My son, and some kid named ''Pete The Villager''. The worst thing is, you need to talk to your wife to complete the main quest!
Now every time i see the game's box, i get a feeling of incompletion, yet i can't be bothered to start a new save.

Oblivion, couldn't stop guarding. As in, it was as if I was holding the right mouse button down when I wasn't. Had to restart 3 times because of it.

Autosaves are gamebreakers for me. Like when a game autosaves right as a guard is giving me a killshot or when the shit is about to hit the fan or right when I would hit the ground after a deadly fall.

I had mine in Fallout 2, where I had gotten to the point where I was going to the final, game-ending area, and all I needed was two McGuffins to get there. I eventually found them both, but I decided that I was gonna finish some more side-quests before I went there.

Fast-forward some hours and I find myself in the control room where I need to place one of aforementioned McGuffins, and I can't find it in my inventory. "Hmm, that's odd", I thought, "I might have left it in the trunk of my car" (/late spoiler alert!). But it wasn't there either. "WTF? Oh wait, did I ever go to that place with those pickpocketing kids while carrying it? I'll go check if they have it."

No luck there either, and at that moment, I realized that the game had glitched, thereby forcing me to start over from the beginning if I wanted to see the ending.
I haven't forgiven it for that quite yet, and I've therefore never played Fallout 2 all the way through.

Damn F.O.D...
