I haven't got a favourite, but I have to say Vlad in GTA4 is ranked toward the top of my list.
I was starting to get tired of his overall arrogance and everything from the car chase to shooting him by the river was, up to that point, the most satisfying part of the game

Dogmeat in Fallout 3. He wasn't as a good as a companion as the dog from Fable, so I just quit wasting stimpacks on him.

God, he was so lame, yet still the game was shoving him in your face like your mother who wanted you to talk to that only other kid at a party your parents had to take you to. Sacrificing him was the quickest decision I ever made in an RPG game.
Seriously? I liked Kaidan, and I think my character did too (something about the way they gazed into each other's eyes). I couldn't stand Ashley. Unfortunately she was always part of my team and I'd spent hundreds of thousands buying equipment for her so I couldn't sacrifice her even though I desperately wanted to. I'm halfway through playing through Mass Effect a second time and this time I'm happy to say she'll get to fuck right off and die.

Oh, and also Francis McReary from GTA IV. That decision took about a millisecond. What a douchebag.
Ashley was obnoxious, but at least she had a great ass.
yeah and she made a good person on my heavy hitter team. (vanguard shepherd, garrus or wrex, and her) although soldier/storm trooper shepherd w/ barrier kind broke the game.

I see people feel bad about killing Jyunichi from SR2, as do I. But, what about Veteran Child? He was just a normal dude, and he really didn't have a choice what to do. Also, Matt from the Brotherhood was just being a loyal friend. i don't think he deserved a brick to the face.

But, I liked killing Eddie Low in GTA IV. Dude creeps me the fuck out.

Martin in Elder Scrolls: Oblivion. I hated his whole messiah complex, he was too much of a goodie-goodie. The only problem is that he's completely glorified upon death.
