Deus Ex (has pretty much its own meme about replaybility)
Kotor I&II played that one so much even after I've add the restored content it feel like already it played it.
San Andreas (mostly for the story mission, but I can't get past

Over the years, I've probably replayed DX more than any other game. The sub-par sequel also gets occasional run-throughs.

I also go through phases of reinstalling old-school point and clickies: the Discworld games, Beneath a Steel Sky, Flight of the Amazon Queen, the Goblins series, and the LucasArts classics, to name but a few.

I've beaten the vast majority of the 100+ games I own more then once. Many more than that. I replay most games at least 5 times because I've learned that, with very few exceptions, the second-and-beyond playthroughs are more enjoyable for me (I think the only exception so far has been Alan Wake).

EDIT: I'm currently playing through the entire Halo series for the first time since Reach came out, but this is probably my 15th or so time beating Halo 1 and 2.

EDIT AGAIN: Also, if I had to guess, I'd say the game I've beaten the most in my life would be The Legend of Zelda: Ocarina of Time. I had to have beaten that at least 40 times.
