While this isn't recent when I played Banjo-Tooie as a kid, I would get freaked the hell out while playing the Underwater level and run into the boss; a giant angler fish. That thing scare the crap out of me.

Only for one time fallout 3 one time me and my friend were exploring the wasteland then i thought i saw something but i thought i was seeing things then i looked behind myself a mole rat already at my face like a face hugger.

While I don't recall myself being scared by a game, my brother in-law gets freaked out in that one part in the first Bioshock where you first enter Rapture and that Spider splicer guts that dude and screams really loud. Though I do admit I jumped a bit when I first heard that.
It wasn't about being scared that the Garys would kill you, it was more the surreality of the whole thing. The Vaults in that game always put me on edge. Political experiments, and their results. Fallout's myriad of backstories really unnerved me.

Ya, I was going to type this but you beat me to it. I am scared of giant creatures underwater for some reason. Pretty sure that is a type of phobia.

Any of the cutscenes in Starcraft with the Zerg. Mainly the one wit the fella in the trench firing like mad, and you see his ammo counter ticking down and you KNOW what's coming....but still. Yea, not the best game for a 5-6 yr old. And wow was that was way back.

Morrowind. The only places that scare me are the Sixth House Bases.

I agree with Outcast on this one. They had enemies that looked like axe-weilding babies that wad a really high-pitched laugh "WAHAHAHAH" there was a whole level full of those things.

This game when I was a little kid.
The video doesn't make it seem all that scary but I was shaking in my kiddy boots at the time.

When I was a little kid (around 4 or 5 maybe) and played Tekken 2, Devil used to scare the crap outta me.

I still remember the day when I forced my mom to sit and watch as I fought him so I wouldn't run off, and finally beat him for the first time. After that, I was never scared of him again, but I still remember that sense of accomplishment when I first got him.

The moon from Majora's Mask - it still freaks me out today!

Also the Two Towers PS2 game - there was one part where you're just walking through a swamp or something and these orcs pop out of nowhere and the music goes DA DUUUUUUUN!!!! I think it was mostly the loud blast of music that scared me. >_<

I did have some game that creeped me out, but I can't recall it for the time being, so I'll post this instead:

EDIT: Star Fox Adventures! The Redeye king or whatever-the-hell that T-Rex thing was scared the crap out of me when it appeared BEHIND me. I was waiting for it to turn the corner when I suddenly heard it roar and saw air waves coming from behind.

That aside exploring the other vaults and my first run in with a Deathclaw in Fallout 3 were quite frightening.

Okay, perhaps I missed it but I am surprised this thread got so far without one mention of the TAILS DOLL from Sonic R.

That thing spawned one of the more interesting urban legends surrounding gaming, and it exploded into a meme of sorts from there on out.

Strictly speaking I think that wins creepiest hands down, simply because it captured the minds of so many people one way or another.

One of the more interesting things about it is that while "debunked" in numerous places, there have been enough ongoing reports to keep it alive. Making it akin to say "The Jersey Devil" more than say a "Candle Cove" where the creators themselves admitted it was bogus and it was tracked down to a single originating point (Ichor Falls), which to my knowlege never happened with this specific urban legend. (queue creepy music).

L4D freaked me out at first when I had to fight Tanks... Now I treat that as a sport or something.

In the past... Metroid Prime games wouldn't scare me, but I'd be on the edge of my seat during certain areas like Phendrana Drifts before I got the Plasma Beam or Torvus Bogs before I got the Annhilator Beam.
