Half-Life, when you first visit the other planet or whatever. The lights go off and then you see flashes of the green creatures that shoot lightnings. That was the first scary thing in a video game that has stayed in my memory.

Also, headcrabs before you get used to them.

Fallout 3. Between the Gary vault and the Dunwich Building, I was genuinely afraid.

And don't even get me started about the Deathclaw Sanctuary.
I don't know about the rest, but I'm pretty sure the Dunwich Building is supposed to be scary, being based on Lovecraft and all...

OT, my problem is generally the opposite, i.e., games that are supposed to be scary which fail to impress me. The most notable case, I think, was F.E.A.R.

But I remember an old MSX game called Science Fiction, where you were in control of a small ship whose shots resembled little spermatozoids battling a huge monster which kinda looked like an uterus. The goal in that game was to blast your way thgourg a vagina-like tube leading to the monster's "brain", which obviously resembled an egg. I swear I'm not making any of this up.

Anyway, my copy of that game probably had some sort of glitch, because I killed that monster hundreds of times before without it emmiting a sound. But one night, as my little tadpole hit the "brain" and the monster "died" (or was impregnated, who can say?), it said in a very bad synthesized voice: "I'll.. be... back!"

I turned my MSX off right then. It took me nearly a month to try and load that game again.

This. Though my situation at Dunwich is different. When I went there, I had outdated addons that conflicted with eachother. Every feral ghoul was really slow, did like no damage but had tons of health. Then the glowing one came. 9 or so bottlecap mines barely did IIII of his health. He also ran as fast as a deathclaw. It was a nightmare!

Fallout 3 is one of those games that was a bit scary to explore the first time, because you never knew where you might end up. My first play through, I ended up in the Yao Goi Tunnels. Boy, that was a wrong turn. Thankfully I had Jericho to liven the mood with his profanity.
Jericho died for me somewhat early, so I didn't get to hear him serenade me with profanity.

I vividly remember him saying ''I'm going to **** your shit up!''
That quote rings a bell. I also remember he had an odd sounding voice actor. The voice actor had a much higher voice than I thought Jericho would have.

The legend of Zelda Ocarina Of Time and Majora's mask both scared me when it came to the water temples, I was afraid of deep water as a kid. Oh and the forest temple from Ocarina of Time.

Lavender Town scared me shitless. Anima in FFX, and fighting her for the first time almost caused me to quit playing. Fuck the laughing, dread Anima's Overdrive.

Unfortunately, I didn't play FFtactics as a kid, but if I did this song would scare me even worse than Lavender town:

Another one is Chicken Run for the Playstation. That dog growling used to freak me the hell out.
Damn, you too? I nearly forgot about that game, I never even finished the first level.

Pikmin 2 was quite gnarly, but it only became actually scary in the submerged castle. Because as I was happily wandering around looking at things all of a sudden some surreal entity called a water wraith came out of nowhere and bulldozed half my Pikmin hoarde. And then it chased me mercilessly through a huge labyrinth.
Holy crap, that thing freaked the Hell out of me. Thankfully, my friend was a hardcore Pikmin fan and told me to hug the walls. That thing couldn't get me then.

So yeah, the Water Wraith in Pikmin 2. That guy was awesome, though.

I' gonna have to go with oblivion on this one. Not terribly scary, but I remember my first Land drough fight.
I entered the cave, and with my Battle hunter gauntlets with life detect in 60ft, I took a look around to see what was awaiting slaughter. A couple mud crabs, sure. A few daedroths, not looking forward to those. A ghost or two, goo thing I brought my sunfire sword. And a...await, what the hell is that shape? It's freakin HUGE! It looks o be....human? No, it seems to have crab leg. Well, maybe it's just 2 enemies standing next to each other.
2 minutes later.
Immergesfrom water.
"ahh, that was a refreshing swim. Now those two enemies should be aroun.....oh. my. god. RUNAWAY!
( I did finally work up the courage to kill it, but still, finding enemies you don't know how to kill on hard mode is not a good thing.)
