I will have to say World at War on veteran, on the approach to the reichstag. after many cheap deaths, i managed to take out two of the four anti-tank guns. the most annoying part of the game is that i can't walk three feet without six fucking grenades falling at my feet.
my other aggrivating game is bayonetta, the levels in paradiso on hard. some of the paradiso monsters are just dead set against me activating witch time.

I've never thrown a controller, but a while ago I was playing Mario Kart and yelled GAAAAAAAAAAAAAAAAAAAAHHH!!! when I got hit by a red shell, blue shell, lightning and POW block one after the other.
I still came 8th but I was miles ahead before I got hit

I used to have a pile of dead mice that resulted from playing Counter-Strike over a modem connection.

To many to recount for me because i have a lot have rage from games. most of which goes towards, the character not following my directions, enemies that spawn from a space i cannot see and Glitches.
Sports games, mostly. Specifically EA's NHL hockey series. I have been playing that series since 1993. My buddy and i are both perfectionists. We want our players leading the league in EVERY stat category... The problem? Your goaltending in these games always, repeat, always blows.
No matter how much you may dominate a game, your goaltending will always give up the absolute cheesiest goals to wreck shutouts and blow your Goals-Again-Average out of the water. Normally they are lofted from center-ice. Many a controller has felt both of our wrathes.
Aside from that it's usually space combat sims like "Wing Commander" or "X-Wing". The difficulty level on some of those is just ridiculously high.

Smash Bros Melee, when try out new combos and an enemy would deny me for the upteenth time........ that caused controller throwing frustration.
Back in the day Donkey Kong Country did, but then I was maybe 7. Haven't done it since (and no I didn't break anything).
I think the last time that happened was back in 2007 where I used to be obsessed with call of duty 3 and it's multiplayer. I was playing that sucker every day and I eventually reached #58 (sniper) on the leader-boards. So naturally I wanted to keep my place and maybe even move up in the ranks,however, their was one day where I was having a very, VERY bad day. Idk what it was but I just wasn't on my game and everyone seemed to be owning me. So this led me to throw my 360 controller across the room, I then found out that my throw was so hard that one of the analog sticks broke. After that I stopped playing games for hardcore competition, sure it's fun but for it to get to me like that I knew I needed to change my ways, and I did.

Dead or Alive 4. Not from the game itself, but the way my brother plays with Jet Li. He actually picked him because of the annoying noises he makes (seriously, he said so). I threw the controller at him. Does that count?
