I don't know if it was tracking it but I beat Final Fantasy X after having gotten everyone's ultimate weapons, all the Aeons, and beating down Omega (Combined Overdrive on the three sisters made it a breeze). So I consider the game beaten 100%

Back in College I was obsessed with Castlevania: Symphony of the Night and finished that having explored 202% of the castle.

I also beat Final Fantasy VI pretty hard my last time through. I'd gotten all the magicite, my favorite characters knew dang near all the spells in the game (seriously you should have seen Sabin's spell list). I even "cured" the cursed shield and successfully wagered the Ragnarok sword at the arena.

More recently I'm only two achievements away from getting 100% on Godfather 2, but I'm in no hurry to do that.

I'm also one run away from having all the achievements for Fallout 3.

I had C&C 3 beat on PC all gold medals with all intels and bonus objectives completed then my computer crashed haven't taken the time to do it again (mainly because I don't have the free time I had when that came out)

I've beaten Super Mario Galaxy 100% as Mario (not Luigi, but I've beaten the hardest levels as him). I've also obliterated Metroid Prime 3 at exactly 10 hours playtime. It was cool seeing only 1s and 0s as the numbers. I've beaten Tales of Symphonia 100% 3 times(new game+, woot). Those are all I remember. There are more.

EDIT: I've beaten three Castlevania games to max: Harmony of Dissonance(200%), Aria of Sorrow(100%), and Portrait of Ruin(all 1000% of it).

Spyro 1: Which I have cmpleted so many times Ican do the entire game under 3 hours to 100%, it was my childhood
Fallout 3: All side quests, bobble heads, expansions, weapons, locations and perk it is possible to get
Guitar hero 3: Every single campaign done, not full 1000 though because of co-op and online achievements which i despise
Disgaea: spent 500 hours on that game, I got so powerful I did 9 billion damage, was the overlord of terror, finished multiple times with every ending, all my weapons on every character had been down the item world to level 100 and I also had a some form of every single person and creature
Resident evil 4: Finished on all difficulties, found every easter egg, finished adas version and had full health by the end too
Finally DMC3: Finished on every difficulty which too about 20 hours, upgraded every weapon to max, had max health and devil mode magic and could remember most weapons specials off the top of my head
...God I just realised how sad I am

Jump Ultimate Stars: Every challenge beat, every koma found, and every single koma's aspects totally unlocked.
