I don't know if everyone has one, but growing up in a house where gaming was prevalent, I was exposed to rather adult games as a kid, and two games stand out that scared me shitless.

1. Diablo- the first one, I think it was the music, isolation, and monster noises. Especially the noise you make when you die.

2. Warlock (SNES)- just look at the freaking intro, and listen to the music! God that scared me. I don't think I ever made it past the third level.

Donkey Kong 64. Those freggin' caves and beavers that respawn at just the right times to scare the crap out of ya. I always found Rarware N64 platformers scary in general. I don't know why.

There was this one old PsOne game that my dad used to play. You flew a helicopter, and I remember at one point you had to rescue glowing irradiated people.

I don't even remember the rest of the gameplay, or what it was called. I just remember being afraid of that mission. It made me paranoid of radiation poisoning for a good year. I was, like, four.

That, and the music accompanying the giant fire-ants in the PsOne Command and Conquer games. Not the ants themselves, though. : )

Star Wars: Shadows of the Empire.

The Sewer level, oh god those monsters scared me. And then I had to fight one giant one while underwater.

Alien Trilogy. I just couldn't play it, I was terrified. it probably had something to do with watching the first Alien film at around 7. It wasn't even 3D! The enemies were sprites but something about the soundtrack, the random noises, the atmosphere. I just couldn't play it.

Resident Evil: The Jill Sandwich version.

I was too scared to play that game for years. Now, not so much.

I used to be afraid of the first Halo, on the first level!

Later got through it, reached the Flood, and got freaked again. Combat Floods carrying rocket launchers are a terrifying sight.

Banjo-Kazooie.

One: Whenever you ended the game, that cutscene of the bad guys winning scared me.
Two: Mazes
Three: That goddamned water level. I hated water levels as a kid.

Legend of Zelda: Ocarina of Time

The Redeads...the first thing I did once I learned about it was get the suns song...by running right through the acid to avoid the ones that guarded it.

Medieval 2

That ripper person scared me when I first encountered him. I was terrified of going outside at night.

I remember Duke Nukem 3D, there was a section that used to grab the pants off of me. And that was every single section that had a facehugger in it :p

So the first chapter was the only chapter I would play unless my oldest brother was in the room with me >.<

Also, Clive Barkers Undying: I remember in the demo when you were inside the mansion, and there was this hallway with a mirror, and you heard laughter, turned around and this dude was right behind you.
