Skies of Arcadia: Legends. The bottom of the sky/sea/whatever. A swirling maelstrom of chaos, and you have to go into it for one part of the game. It still freaks me out.

There was a weird game for the Sega Genesis that I rented a few times, and it scared the heck out of me every time! Thing is I can't remember the name.

You were this weird green poltergeist thing, living in a families house. Now, the family couldn't see you, you would walk amongst them and they would have no idea you were there. The objective was to scare the family out of the house, one by one, by controlling household objects to do weird paranormal stuff, thereby scaring the living daylights out of the family members!

I remember that this cost energy, and your energy also slowly decreased with time. You refilled your energy by picking up green slime. If you ran out of energy, you would be sent to what I can only assume must be the underworld? There was a bunch of green slime there, and you refilled your energy and went back. Maybe there were things that could kill you there? Maybe. Anyway, that was the first thing that I found frightening about this game.

The first couple levels were easy, then there were enemies in the house. Flying demonic heads kinda enemies. Scared the bejeezus out of me every time, and I wouldn't play anymore (come on, I was like 4 or 5). They would do is drain your energy, and send you to the underworld, which sucks because I was scared of that too!

Part of the reason I posted this in the hope that someone could tell me the name of this game. I'd like to give those demonic heads something they've had coming to them for 20-odd years!! Thanks!

The Great Giana Sisters (on Amiga). Not the game itself, but some of those creatures were scary. At least the bosses. Also Rick Dangerous. Those traps (you can for the most part only find through trial and error) and screams and intensive situations made it. RD II was more scarier of the two though.

Quake was pretty scary too, as well as Doom.

Zelda II. There were a couple places in the final palace where a jumbo slime monster dropped from the ceiling. Yeah, they scared me because I had no idea they were there.

Game was called 'Haunting: Starring Polter-guy'. You were a skater who was killed by a businessman's shady practices so you decided to make him suffer some. Actually was a real fun game.

Edit: Bah, beaten to the punch and with a wiki article no less.

Duke Nukem 3D. I remember playing Duke Nukem and there was a woman wrapped in slime and me and my brother faintly heard "you can kill meee" from what we assumed was the woman. Scary stuff.

Friday the 13th for the NES, had a habit of doing the jump-scare at exactly the right time. I was 8 years old and having Jason show up out of nowhere with a loud noise was creepy as hell.

I have always been scared of the series The House of the Dead. I vowed never to play that game.

....And now it is my favorite Sega series now. Long live the Magician!

I remember getting a Silent Hill demo when I bought MGS for the Playstation.

I couldn't get colour on the tv, but decided to try it out anyway. It was a section in the school, all I had was a torch and hadn't really figured out the controls. I tried walking around a bit and then suddenly shone the light on something crawling towards me, dragging it's body along with its hands.

Never touched it again.

I wasn't even scared of Doom and never played a survival horror game, so none.

It was my very first gaming experience. My dad sat me infront of Doom (on DOS) and a giant brain on cyborg spider legs startled me into a corner. It proceded to fire green shit at me and make loud robot noises until I fell with a bloodcurdling scream (my dad turned the volume to max), I proceeded to cry.

Doom 3 gave me the creeps too... so much dark.
Ah yes, the arachnatron. I can only managed what the spider mastermind would've been like for you.
