Majora's Mask. That game still scare's the hell out of me.
Look up something called "Ben Drowned". Bricks will be shat.
OT: Oh yes...the videos' are filled with stupid shocker scares though. If done correctly it could have been a very good creepypasta.

Now I think the game that scared me a while back was Alone in the dark: Revenge of something or other, for the sega saturn. Also the sonic drowning music. Also a few parts in shining wisdom. Also, like said above, the eel from mario 64 and the Well level from OOT. LoZ: MM was pretty eery for some reason as well, good thing that the "ben drowned" thing wasnt made up back then or I would have shat myself.

The legend of Zelda games for the nintendo 64. Until the age of 8. Shut up.
I hear you. The re-deads and wallmasters scared the hell out of me. Which is why I had trouble beating Forest Temple, Shadow Temple (I wouldn't be surprised if I still have nightmares of that place), and Spirit Temple.

Resident Evil 4. I bought it and was too afraid to play it at night. Had to play it in the daytime.
Somewhat this, although replace Resident Evil 4 with F.E.A.R, still haven't beaten them... (albeit, I try to man up every now and again to play it at like 3 or 4 in the morning when i'm by myself. Usually gets a little unnerving so I quit)

I would say the Jurassic Park game for SNES. scared the crap out of me as a child. Now that i see screenshots of it, idk why i was. XD

I would say the sounds and atmosphere, my young age, and the fact that it was first person perspective that did it. And I played it in the dark.

Castlevania I & IV

Took me forever to get through IV, never beat the first Castlevania, though, and probably never will be, since I'll never be as good at that game as I was when I first played it when I was 7 or so

Boss battles in Donkey Kong Country, SNES. Tons of anxiety with those for some reason.

And, I should probably get checked out for this, but playing bomberman 64 recently, I was EXTREMELY anxious, cold sweats, very high heart rate (not fat) and everything. Granted, I played/got immensely frustrated with/loved this game as a kid...This is slightly off topic (lol).

Any FPS... Yes I know that is sad but I would always go super slow through the levels and I would slowly peak around a corner searching for enemies.

Rayman 2: The Great Escape... Shut up and don't judge me, it was just this one level ok?

- Edit -

You can see why this would be scary to a six-year-old right?

Oh, and i just remembered... Harry Potter and the Chamber of Secrets: The Game... Again, only one level... God, i was scared of these, but not Resident Evil 1 or 2... I was a weird kid...
